摘要
目的探析开颅手术麻醉中应用头部神经阻滞复合静脉全麻的临床应用效果。方法该文观察对象选自该院2016年1月—2019年12月收治的86例开颅手术患者,以不同麻醉方式为依据,对86例患者进行分组,对照组43例患者接受静脉全麻处理,观察组43例患者基于对照组的静脉全麻处理,采用头部神经阻滞麻醉,对比两组疼痛情况麻醉药物使用量差异情况。结果观察组患者术后2、6、24 hVAS评分分别为(2.38±1.87)分、(3.15±2.32)分、(3.23±1.42)分,均低于对照组,差异有统计学意义(t=5.882、2.866、4.293,P<0.05);观察组术后48 hVAS评分为(4.68±1.62)分,与对照组相比差异无统计学意义(t=0.028,P>0.05);观察组患者瑞芬太尼用量(1297.6±563.2)μg、丙泊酚用量(1220.3±529.6)mg,明显少于对照组,差异有统计学意义(t=6.667、2.122,P<0.05)。结论开颅手术中应用头部神经阻滞复合静脉全麻处理,患者麻醉效果佳,由于术后镇痛效果好,是开颅手术患者首选的一种麻醉方式,推广应用价值较高。
Objective To explore the clinical application effect of cranial nerve block combined with general venous anesthesia during craniotomy.Methods The observation objects in this article were selected from 86 patients undergoing craniotomy in the hospital from January 2016 to December 2019.Based on different anesthesia methods,86 patients were divided into two groups,and 43 patients in the control group received intravenous general anesthesia.Based on the intravenous general anesthesia of the control group,43 patients in the observation group were treated with cranial nerve block anesthesia,and the difference in the use of anesthetic drugs for pain between the two groups was compared.Results The VAS scores of patients in the observation group were(2.38±1.87)points,(3.15±2.32)points,and(3.23±1.42)points at 2,6,and 24 h,respectively,which were lower than those of the control group,and the difference was statistically significant(t=5.882,2.866,4.293,P<0.05);The 48 h VAS score of the observation group was(4.68±1.62)points after surgery,which was not statistically different from the control group(t=0.028,P>0.05);the dosage of remifentanil in the observation group was(1297.6±563.2)μg The dosage of propofene(1220.3±529.6)mg were significantly less than that of the control group,and the difference was statistically significant(t=6.667,2.122,P<0.05).Conclusion The application of cranial nerve block combined with intravenous general anesthesia in craniotomy surgery has good anesthesia effect for patients.Because of the good postoperative analgesia effect,it is the preferred anesthesia method for patients with craniotomy,and it is of high value for promotion and application.
