地黄骨痛康胶囊联合低频脉冲导入双氯芬酸治疗膝骨性关节炎的效果观察

Effect of Dihuang Gutongkang Capsule combined with Low Frequency Pulse Introduction of Diclofenac on Knee Osteoarthritis

目的探讨地黄骨痛康胶囊联合低频脉冲导入双氯芬酸治疗膝骨性关节炎的效果。方法选取2018年8月-2019年7月医院收治的68例膝骨性关节炎,按随机数字表法分为观察组和对照组各34例。对照组采取玻璃酸钠注射液关节腔注射,观察组在此基础上加用地黄骨痛康胶囊联合低频脉冲导入双氯芬酸二乙胺乳胶剂治疗。两组患者1个疗效为7 d,连续治疗3个疗程。评估两组临床治疗效果以及治疗前后疼痛程度,中医症状积分,膝关节功能,以及肿瘤坏死因子(TNF-α)、白介素-1(IL-1)和白介素-6(IL-6)等炎性因子水平。记录两组不良反应。结果观察组患者治疗总体有效率显著高于对照组(P<0.05)。两组治疗后疼痛数字(NRS)评分、中医症状积分、TNF-α、IL-1和IL-6均显著低于治疗前(P<0.05),观察组患者NRS评分、中医症状积分、TNF-α、IL-1和IL-6等均显著低于对照组(P<0.05)。两组治疗后膝关节功能评分高于治疗前(P<0.05),观察组患者膝关节功能评分显著高于对照组(P<0.05)。两组患者均未出现严重不良反应,两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论地黄骨痛康胶囊联合低频脉冲导入双氯芬酸治疗膝骨性关节炎疗效及安全性均较好,不仅能有效提高膝关节功能,同时还能缓解关节疼痛及改善炎性反应。

Objective To explore the effect of Dihuang Gutongkang capsule combined with low frequency pulse introduction of diclofenac in the treatment of knee osteoarthritis.Methods A total of 68 cases of knee osteoarthritis admitted to our hospital from August 2018 to July 2019 were selected and divided into study group(n=34)and control group(n=34)according to the random number table method.In the control group,patients were injected with sodium hyaluronate into articular cavity.In the study group,patients were given Dihuang Gutongkang capsule combined with low frequency pulse introduction of diclofenac diethylamine emulsion on this basis.One therapeutic effect of two groups was 7 days,totally three courses of continuous treatment.The clinical treatment effect,pain degree,TCM symptom score,knee joint function and levels of tumor necrosis-α(TNF-α),interleukin-1(IL-1)and interleukin-6(IL-6)of the two groups were evaluated before and after treatment.Adverse reactions were recorded in two groups.Results The total effective rate of the study group was significantly higher than that of the control group(P<0.05).The NRS score of pain,TCM symptom score,levels of TNF-α,IL-1 and IL-6 in the two groups were significantly lower than those before treatment(P<0.05),and those in the study group were significantly lower than those in the control group(P<0.05).The score of knee joint function in the two groups after treatment was higher than that before treatment(P<0.05),and that in the study group was significantly higher than that in the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Dihuang Gutongkang capsule combined with low frequency pulse introduction of diclofenac in the treatment of knee osteoarthritis has good efficacy and safety.It can not only effectively improve the knee joint function of patients,but also relieve joint pain and improve inflammatory response of colleagues.
