脐静脉置管序贯PICC在极低出生体重早产儿的应用

Application of Umbilical Vein Catheterization Sequential PICC in Very Low Birth Weight Premature Infants

目的探讨脐静脉置管序贯PICC在极低出生体重早产儿的应用效果。方法方便选取2018年12月—2020年9月期间的82例极低出生体重的早产儿为研究对象,分为导管组44例(UVC序贯PICC)和非导管组(外周静脉补液组)38例。分别对比两组的住院时间、恢复出生体重时间、并发症的发生率以及院内感染率。结果导管组患儿平均住院时间和恢复出生体重时间分别为(38.2±11.9)d、(5.9±1.2)d,均低于非导管组,差异有统计学意义(t=3.502,3.514 P<0.05)。导管组屏气发作次数(32.1±14.1)次低于非导管组(55.8±14.0)次,差异有统计学意义(P<0.05);导管组屏气发作次数为(32.1±14.1)次,少于非导管组的(55.8±14.0)次,差异有统计学意义(t=7.615,P<0.05)。其他早产儿的相关并发症的发生率在两组间差异无统计学意义(P>0.05)。结论脐静脉置管序贯PICC在极低出生体重早产儿的应用效果显著,值得新生儿科临床推广。

Objective To investigate the effect of sequential PICC with umbilical vein catheterization in very low birth weight preterm infants.Methods Eighty-two preterm infants with very low birth weight between December 2018 and September 2020 were the study subjects.They were divided into 44 cases in the catheter group(UVC sequential PICC)and 38 cases in the non-catheter group(peripheral intravenous fluid replacement group).The length of hospital stay,time to birth weight,incidence of complications,and nosocomial infection rates were compared between the two groups.Results The average hospitalization time and the time to recover birth weight of the catheter group were(38.2±11.9)d and(5.9±1.2)d,respectively,which were lower than those of the non-catheter group.The difference was statistically significant(t=3.502、3.514 P<0.05).The number of breath-holding episodes in the catheter group(32.1±14.1)times was lower than that in the non-catheter group(55.8±14.0)times,and the difference was statistically significant(P<0.05);The number of breath-holding episodes in the catheter group was(32.1±14.1)times,which was less than(55.8±14.0)times in the non-catheter group.The difference was statistically significant(t=7.615,P<0.05).related complications in other preterm infants,there was no significant difference in the incidence rates between the two groups,and the difference was not statistically significant(P>0.05).Conclusion The use of sequential PICC with umbilical vein catheterization in preterm infants with very low birth weight has significant effects,which is worthy of clinical promotion in neonatal department.
