25羟维生素D与2型糖尿病合并高血压的相关分析 被引量:4

Correlation analysis of 25 hydroxyvitamin D and type 2 diabetes mellitus complicated with hypertension

摘要 目的探讨25羟维生素D[25(OH)D]与2型糖尿病合并高血压的关系。方法 84例2型糖尿病患者,根据病情不同分为2型糖尿病合并高血压组(34例)及单纯2型糖尿病组(50例),另将体检中心提供的32例健康人群设为对照组。所有受试者均测量身高、体重、血压、腰围,检测25(OH)D、血糖、糖化血红蛋白、血脂,分析25(OH)D与2型糖尿病合并高血压的关系。结果三组身高比较,差异无统计学意义(P>0.05);对照组的体重、血压及腰围均低于2型糖尿病组及2型糖尿病合并高血压组,差异有统计学意义(P<0.05);2型糖尿病合并高血压组的体重、血压及腰围均高于2型糖尿病组,差异有统计学意义(P<0.05)。对照组空腹血糖、糖化血红蛋白、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)及25(OH)D水平均低于2型糖尿病组及2型糖尿病合并高血压组,高密度脂蛋白胆固醇(HDL-C)高于2型糖尿病组及2型糖尿病合并高血压组,差异有统计学意义(P<0.05);2型糖尿病合并高血压组空腹血糖、糖化血红蛋白、TG、LDL-C及25(OH)D水平均高于2型糖尿病组, HDL-C低于2型糖尿病组,差异有统计学意义(P<0.05)。三组女性25(OH)D水平均低于本组男性,差异具有统计学意义(P<0.05)。25(OH)D与血糖、血压、血脂、糖化血红蛋白呈负相关(r=-0.123、-0.013、-0.583、-0.325, P<0.05)。多因素分析可知, 25(OH)D降低是2型糖尿病合并高血压的独立危险因素(P<0.05)。结论糖尿病及糖尿病合并高血压患者更容易存在维生素D缺乏及不足,且女性患者存在更多的维生素D缺乏与不足,提示生素D不足可能是糖尿病及糖尿病合并高血压的独立危险因素。 Objective To discuss the correlation between 25 hydroxyvitamin D[25(OH)D]and type 2 diabetes mellitus complicated with hypertension.Methods A total of 84 type 2 diabetes mellitus patients were divided into type 2 diabetes mellitus complicated with hypertension group(34 cases)and simple type 2 diabetes mellitus group(50 cases)according to different conditions of disease.32 healthy subjects were taken as the control group.The height,weight,blood pressure,waist circumference of the three groups were measured,25(OH)D,blood glucose,glycosylated hemoglobin,and blood lipids were detected,and the correlation between 25(OH)D and type 2 diabetes mellitus complicated with hypertension was analyzed.Results There was no statistically significant difference in height of the three groups(P>0.05).The weight,blood pressure and waist circumference of the control group were lower than those of the type 2 diabetes mellitus group and the type 2 diabetes mellitus complicated with hypertension group,and the difference was statistically significant(P<0.05);the weight,blood pressure and waist circumference of the type 2 diabetes mellitus complicated with hypertension group were higher than those of the type 2 diabetes mellitus group,and the difference was statistically significant(P<0.05).The fasting blood glucose,glycosylated hemoglobin,triglyceride(TG),low density lipoprotein cholesterol(LDL-C)and 25(OH)D in the control group were lower than those in the type 2 diabetes mellitus group and type 2 diabetes mellitus complicated with hypertension group,and the high density lipoprotein cholesterol(HDL-C)in the control group was higher than that in the type 2 diabetes mellitus group and the type 2 diabetes mellitus complicated with hypertension group,and the difference was statistically significant(P<0.05);the fasting blood glucose,glycosylated hemoglobin,TG,LDL-C and 25(OH)D in type 2 diabetes mellitus complicated with hypertension group were higher than those in type 2 diabetes mellitus group,while HDL-C was lower than that in type 2 diabetes mellitus group,and the difference was statistically significant(P<0.05).The 25(OH)D levels of women in the three groups were lower than men in those groups,and the difference was statistically significant(P<0.05).25(OH)D was negatively correlated with blood glucose,blood pressure,blood lipids,glycosylated hemoglobin and other indicators(r=-0.123,-0.013,-0.583,-0.325;P<0.05).Multivariate analysis showed that 25(OH)D was an independent risk factor for type 2 diabetes mellitus complicated with hypertension(P<0.05).Conclusion Vitamin D deficiency is more likely to exist in patients with diabetes and hypertension,and more vitamin D deficiency exists in female patients,suggesting that vitamin D deficiency may be an independent risk factor for diabetes and diabetes complicated with hypertension.
