聚焦解决模式对老年呼吸衰竭患者心理状态和生活质量的影响

Analysis of the effect of focused resolution mode on mental state and quality of life in elderly patients with chronic respiratory failure

目的观察聚焦解决模式对老年呼吸衰竭患者心理状态、生活质量的影响。方法将2017年1月至2019年1月该院收治的110例老年慢性阻塞性肺疾病合并Ⅱ型呼吸衰竭患者随机分为观察组和对照组,每组55例。两组均给予无创呼吸机治疗。对照组在治疗的同时接受常规护理,观察组采用聚焦解决模式5个步骤进行健康教育及心理护理,其他护理与对照组相同。比较两组患者护理前后疾病相关知识知晓情况、焦虑、抑郁、生活质量、血气分析指标等。结果观察组患者护理后疾病相关知识知晓率、躯体、认知、情绪、角色、社会功能、总体健康评分、动脉血氧分压、氧合指数(动脉血氧分压/吸入氧气分数)均明显高于对照组,差异均有统计学意义(P<0.05)。两组患者护理后焦虑自评量表、抑郁自评量表评分均明显低于护理前,且观察组护理后焦虑自评量表、抑郁自评量表评分、动脉血二氧化碳分压均明显低于对照组,差异均有统计学意义(P<0.05)。结论对老年呼吸衰竭患者在常规护理基础上给予聚焦解决模式进行健康教育及心理护理可改善患者不良心理状态,提高其生活质量,改善肺功能。

Objective To observe the effect of focused resolution mode on mental state and quality of life in elderly patients with chronic respiratory failure.Methods A total of 110 elderly patients with COPD and typeⅡrespiratory failure were randomly divided into control group and observation group.Both groups were treated with non-invasive ventilator.The control group received routine care at the same time as the treatment.The observation group used the focused solution mode for 5 steps for health education and psychological care.The other care was the same as the control group.The knowledge of disease-related knowledge after treatment,anxiety,depression,quality of life,and improvement of blood gas index were compared.Results The awareness rate,body,cognition,emotion,role,social function,overall health score,PaO 2,PaO 2/FiO 2 of the observation group were higher than those of the control group(P<0.05).After nursing,the SAS and SDS scores of the two groups were lower than those of the same group before treatment(P<0.05).After the nursing,the SAS,SDS score and PaCO 2 of the observation group were lower than those of the control group(P<0.05).Conclusion For elderly patients with chronic respiratory failure,focusing on the mode of solving the health education and psychological care on the basis of singing care can improve the patient′s bad mental state,improve the quality of life and improve lung function.
