全髋关节置换术与半髋关节置换术治疗老年股骨颈骨折临床价值的比较研究

Comparison of the clinical value of total hip joint treatment and half hip arthroplasty in the treatment of senile femoral neck fracture

目的探讨全髋关节置换术与半髋关节置换术治疗老年股骨颈骨折的临床价值。方法选取2017年6月至2019年6月该院骨科收治的老年股骨颈骨折患者130例作为研究对象,根据治疗方式不同分为观察组(实施全髋关节置换术)和对照组(实施半髋关节置换术),每组65例。比较两组患者治疗后髋关节功能评分及影像学指标。结果观察组患者术后3、6个月Harris评分,以及术后6个月髋臼完好率均明显高于对照组,术后髋臼磨损率、增生率均明显低于对照组,差异均有统计学意义(P<0.05);两组患者假体松动率比较,差异无统计学意义(P>0.05)。结论在老年股骨颈骨折的治疗中全髋关节置换术后髋关节功能改善情况优于半髋关节置换术,髋臼完好率更高,有利于患者术后生活质量的恢复,值得临床推广应用。

Objective To explore the clinical value of total hip arthroplasty and hemiarthroplasty for elderly femoral neck fractures.Methods From June 2017 to June 2019,130 elderly patients with femoral neck fractures in our hospital′s orthopedics were selected as the research object,and according to different treatment methods,they were divided into a control group(performing hemiarthroplasty)and an observation group(performing total hip arthroplasty),comparing the hip function score and imaging indexes of the two groups after treatment.Results The Harris score of the observation group was significantly higher than that of the control group 3 months and 6 months after operation(P<0.05);the acetabular intact rate of the observation group was 89.23%,which was higher than that of the control group 61.54%(P<0.05),the acetabular wear rate and hyperplasia rate were 6.15%,3.08%were lower than the control group 20.00%,13.85%(P<0.05),the prosthesis loosening rate comparison between the two groups of patients There was no significant difference(P>0.05).Conclusion In the treatment of femoral neck fractures in the elderly,the improvement of hip function after total hip arthroplasty is better than that of hemiarthroplasty,and its acetabular intact rate is higher,which is conducive to the recovery of the quality of life of patients after surgery and is worthy of clinical Promotion and application.
