孕足月初产妇应用宫颈扩张球囊与地诺前列酮栓促宫颈成熟的作用比较

Comparison of cervical dilation balloon and dinoprostone suppository in promoting cervical ripening in full term primipara

目的:比较孕足月初产妇应用宫颈扩张球囊与地诺前列酮栓促宫颈成熟的作用。方法:2018年4月-2020年4月收治初产妇60例,随机分为两组,各30例。对照组予以宫颈扩张球囊促宫颈成熟;试验组予以地诺前列酮栓促宫颈成熟,比较两组初产妇妊娠结局。结果:两组治疗前及治疗后Bishop评分比较,差异无统计学意义(P>0.05)。两组促宫颈成熟效果比较,差异无统计学意义(P>0.05)。试验组自然临产率及24 h阴道分娩率均显著高于对照组,差异有统计学意义(P<0.05)。试验组临产时间及产程时间均显著短于对照组,差异有统计学意义(P<0.05)。两组并发症发生率及新生儿窒息发生率比较,差异无统计学意义(P>0.05)。结论:宫颈扩张球囊与地诺前列酮栓在孕足月初产妇引产中使用均有较好的促宫颈成熟作用,但应用地诺前列酮栓能够缩短临产时间,加快产程,使得自然临产率及阴道分娩率提高,但需严格把握适应证及禁忌证,根据产妇时间情况予以个体化处理,以保证母婴安全。

Objective:To explore the effect of cervical dilation balloon and dinoprostone suppository in promoting cervical ripening in full term primipara.Methods:From April 2018 to April 2020,60 primiparas were selected and randomly divided into two groups with 30 cases in each group.The control group was given cervical dilation balloon to promote cervical ripening.The experimental group was given dinoprostone suppository to promote cervical ripening.We compared the pregnancy outcomes between the two groups.Results:There was no significant difference in Bishop score between the two groups before and after treatment(P>0.05).In the experimental group,the natural labor rate and 24-hour vaginal delivery rate were significantly higher than those in the control group,the difference was statistically significant(P<0.05).In the experimental group,the labor time and labor process time were significantly shorter than those in the control group,the difference was statistically significant(P<0.05).There was no significant difference in the incidence of complications and neonatal asphyxia between the two groups(P>0.05).Conclusion:Both cervical dilation balloon and dinoprostone suppository in full term primipara induced labor can promote cervical ripening,however,dinoprostone suppository can shorten the labor time,accelerate the progress of labor process,and increase the natural labor rate and vaginal delivery rate,however,indications and contraindications should be strictly controlled,and individualized treatment should be carried out according to the time of puerpera,so as to ensure the safety of mother and infant.
