摘要
目的探讨甲状腺乳头状癌(PTC)颈部中央区淋巴结转移与超声征象的相关性。方法回顾性分析2014年1月至2018年6月195例PTC患者的超声影像与病理结果,根据颈部中央区淋巴结转移情况,分为淋巴结转移组与淋巴结无转移组,比较两组PTC颈部中央区淋巴结转移超声征象的差异。结果单因素分析:两组PTC病灶的前后径、上下径、左右径、病灶形态、PTC是否突向甲状腺包膜、病灶内是否存在微钙化,差异均有统计学意义(P<0.05),均为颈部中央区淋巴结转移的危险因素。多因素分析:PTC病灶突向甲状腺包膜为PTC颈部.中央区淋巴结转移的独立危险因素。结论PTC超声征象可以预测颈部中央区淋巴结转移,能为临床手术方式的选择提供一定影像学依据。
Objective To investigate the ultasonographic features of papillary thyroid carcinoma(PTC)that affect the cervical central lymph node metastasis.Methods The ultrasonographic and pathological findings of 195 patients with paillry thyroid cancer from January 2014 to June 2018 were retrospectively analyzed.According to the lymph node metastasis in the central region of the neck,all cases were divided into two groups:lymph node metastasis group and non-metastasis group.The diferences of the ultrasonographic signs afcting the lymph node metastasis in the central region of the neck between the two groups were compared.Results Univariate analysis showed that the anterior and posterior diameters,upper and lower diameters,left and right diameters,shape,thyroid papillary carcinomas protruding to thyroid capsule,microcalcifcation were sigificantly diferent between the two groups(all P<0.05),and the diference was stistically significant(all P<0.05),were the risk factor for lymph node metastasis in the central cervical region of thyroid pillary carcinoma.Multivariate analysis(Logitic regression analysis)showed that the protrusion of thyroid pillary carcinoma to thyroid capsule was an independent risk factor for lymph node metastasis in the central cervical region of thyroid papillary catcinoma.Conclusion Some ultasonographic features of papillary thyroid carcinoma can be used to predict lymph node metatasis in the central region of the neck,and provide certain imaging basis for the choice of clinical surgical methods.
