超声在0~6个月婴儿发育性髋关节发育不良早期筛查中的应用

Application of ultrasonography in early screening of developmental dysplasia of the hip in infants aged 0-6 months

目的探索超声在发育性髋关节发育不良(developmental dysplasia of the hip,DDH)早期筛查中的应用价值。方法2018年1月至2019年12月对7096例6个月以内婴儿进行髋关节早期筛查,采用Graf超声检查的方法评价髋关节发育情况。结果超声检查发现DDH患儿211例,筛查阳性率为2.97%,包括异常髋关节数293髋,占全体受检髋关节总数的2.06%(293/14192)。在初筛的DDH中,女婴阳性率高于男婴,不同年龄组之间差异也有统计学意义(均P<0.001),此外秋冬季出生婴儿多发。结论在婴儿期开展超声筛查有助于提高DDH患儿的早期诊断率,但新生儿期筛查可能存在较高假阳性。

Objective To explore the value of ultrasonography in early screening of developmental dysplasia of the hip(DDH).Methods From January 2018 to December 2019,early screening of hip joints was carried out on 7096 infants less than 6 months,and the hip joint development was evaluated by Graf ultrasound.Results 211 DDH were found in ultrasonography,and the positive rate was 2.97%,which included 293 hips with abnormal hip joints,accounting for 2.06%(293/14192)of the total hip joints examined.In the primary screening of DDH,the positive rate of female infants was higher than that of male infants,and the difference between different age groups was statistically significant(all P<0.001).In addition,DDH was more often found in infants born in autumn and winter.Conclusion Ultrasound screening in infancy can improve the early diagnosis rate of DDH children,but the neonatal screening may have high false positive.
