益气温阳利水法热奄包热敷治疗气虚血瘀型下肢静脉功能不全临床观察

Clinical observation on treating venous insufficiency in lower extremity of qi deficiency and blood stasis type with hot compress using Chinese medicine hot package through invigorating qi,warming yang,and circulating water

目的观察益气温阳利水法热奄包热敷治疗气虚血瘀型下肢静脉功能不全的临床疗效。方法将62例气虚血瘀型下肢静脉功能不全患者按照随机数字表法分为2组。对照组31例予五苓胶囊治疗,治疗组31例在对照组基础上联合益气温阳利水法热奄包热敷治疗。2组均治疗14 d。比较2组治疗前后疼痛视觉模拟评分(VAS)、外踝周径、髌骨上10 cm周径及髌骨下10 cm周径变化情况,比较2组治疗前后匹兹堡睡眠质量指数(PSQI)评分(包括睡眠质量、睡眠潜伏期、睡眠时间、睡眠效率、睡眠障碍、催眠药物的使用及日间功能障碍)及简明健康状况调查表(SF-36)评分(包括生理机能、生理职能、躯体疼痛、一般健康状况、精力、社会功能、情感职能及精神健康)变化情况。结果与本组治疗前比较,2组治疗后疼痛VAS均明显降低(P<0.05),外踝周径、髌骨上10 cm周径及髌骨下10 cm周径均明显减小(P<0.05),且治疗后治疗组各指标改善均优于对照组(P<0.05)。2组治疗后睡眠质量、睡眠潜伏期、睡眠时间、睡眠效率、睡眠障碍、催眠药物的使用及日间功能障碍评分与本组治疗前比较均明显降低(P<0.05),且治疗后治疗组PSQI各项评分均低于对照组(P<0.05)。2组治疗后生理机能、生理职能、躯体疼痛、一般健康状况、精力、社会功能、情感职能及精神健康评分与本组治疗前比较均明显升高(P<0.05),且治疗后治疗组SF-36各项评分均高于对照组(P<0.05)。结论益气温阳利水法热奄包热敷治疗气虚血瘀型下肢静脉功能不全临床疗效确切,可明显缓解疼痛,促进水肿消退,改善患者睡眠质量,提高生活质量。

Objective To observe the clinical efficacy of treating venous insufficiency in lower extremity of qi deficiency and blood stasis type with hot compress using Chinese medicine hot package through invigorating qi,warming yang,and circulating water.Methods 62 patients with venous insufficiency in lower extremity of qi deficiency and blood stasis type were divided into the control group(n=31)and the treatment group(n=31)according to the random number table.Patients in the control group were treated with Wuling Jiaonang while the treatment group with hot compress using Chinese medicine hot package through invigorating qi,warming yang,and circulating water based on the control group.Patients in the two groups were treated for 14 days.Then the score of Visual Analogue Scale(VAS)and the changes of cross-section diameter of the lateral malleolus,the location at 10 cm above patella,and the location at 10 cm below patella in the two groups were compared before and after treatment.And the scores of Pittsburgh Sleep Quality Index(PSQI)(including subjective sleep quality,sleep latency,sleep time,sleep efficiency,sleep disorders,hypnotics use and daytime dysfunction)and Short Form 36 Health Survey(SF-36)(including physiological function,role physical,body pain,general health condition,energy,social function,role emotional and mental health)in the two groups were compared before and after treatment.Results The post-treatment VAS scores in the two groups were obviously lower than those results before treatment(P<0.05).And the cross-section diameters of the lateral malleolus,the location at 10 cm above patella,and the location at 10 cm below patella were obviously reduced after treatment.The change of each index in the treatment group was superior to the control group(P<0.05).After treatment,the scores of PSQI in the two groups were obviously lower than those results before treatment(P<0.05).And each PSQI score in the treatment group was lower than that of the control group(P<0.05).After treatment,the scores of SF-36 in the two groups were obviously higher than those results before treatment(P<0.05).And each SF-36 score in the treatment group was higher than that of the control group(P<0.05).Conclusion It has a definite curative effect of treating venous insufficiency in lower extremity of qi deficiency and blood stasis type with hot compress using Chinese medicine hot package through invigorating qi,warming yang,and circulating water.And this kind of therapy can significantly relieve the pain,facilitate the dropsy diminishment,improve the sleep quality,and enhance the life quality for patients.
