系统性红斑狼疮中医体质与中医证候关系研究

Research on the Correlation between the Traditional Chinese Medicine Constitution and Traditional Chinese Medicine Syndromes of Systemic Lupus Erythematosus

目的观察系统性红斑狼疮患者中医体质与中医证候分布特征,分析二者之间的关系。方法选取2019年1月—2019年12月收治的90例系统性红斑狼疮患者,进行中医体质问卷调查和中医证候判定,观察分析系统性红斑狼疮患者中医体质与中医证候分布是否存在差异。结果中医体质类型中平和质(42.22%)、阳虚质(28.89%)、阴虚质(11.11%)3种体质占主要比例。中医证候分型以脾肾阳虚证(74.44%)居多,其次为阳虚质(28.89%)、阴虚质(11.11%)。不同体质类型发病后中医证候分布差异具有统计学意义,平和质、阳虚质、血瘀质的患者发病后多数表现为脾肾阳虚证;阴虚质发病后表现为阴虚内热证、肝肾阴虚证;湿热质发病后表现为风湿热痹证和脾肾阳虚证。结论系统性红斑狼疮患者中医体质类型以平和质、阳虚质、阴虚质为主,发病后中医证候的形成受体质类型影响而具有倾向性。

Objective To observe the correlation between TCM constitution and TCM syndromes in patients with systemic lupus erythematosus. Methods Ninety patients with systemic lupus erythematosus were selected as the research objects,and a TCM constitutional questionnaire survey and TCM syndrome determination were performed to observe and analyze the TCM constitution and TCM syndromes of patients with systemic lupus erythematosus. Results Among the 90 patients,there were 7 males and 83 females. The average age of the patients was 38 years,and the average disease duration was 7 years. The main constitutions of TCM constitutions are peaceful constitution( 42. 22%),yang deficiency( 28. 89%),and yin deficiency( 11. 11%). TCM syndrome types are mostly spleen and kidney yang deficiency( 74. 44%),followed by yang deficiency 28. 89% and yin deficiency 11. 11%. The distribution of TCM syndromes after the onset of different constitution types is statistically significant. Most patients with calmness,yang deficiency,and blood stasis have symptoms of spleen and kidney yang deficiency after onset;yin deficiency and internal heat syndrome,liver-kidney yin deficiency syndrome;rheumatic fever syndrome and spleen-kidney yang deficiency syndrome after onset of damp-heat quality. Conclusion The type of TCM constitution in patients with systemic lupus erythematosus is mainly peaceful,yang-deficiency and yin-deficiency. After the onset of TCM syndromes,the formation of the receptor type affects and has a tendency.
