14-3-3η蛋白联合抗MCV抗体检测在RA诊断中的应用

Application of 14-3-3η protein combined with anti-mutated citrullinated vimentin antibody detection in diagnosis of rheumatoid arthritis

目的:探讨14-3-3η蛋白和抗突变型瓜氨酸波形蛋白(Mutated citrullinated vimentin,MCV)抗体在类风湿关节炎患者(Rheumatoid arthritis,RA)血清中的表达情况,评价两者联合检测对RA诊断的临床意义。方法:在同一时间段内收集50例类风湿性关节炎(RA)患者,45例骨关节炎(OA)患者,40例健康对照者(HC),应用酶联免疫吸附实验(ELISA)法检测血清中抗MCV抗体和14-3-3η蛋白的含量,同时与临床常用指标抗环瓜氨酸多肽抗体(anti-cyclic peptide containing citrulline,anti-CCP)进行比较,利用SPSS22.0软件对实验数据进行统计分析。结果:RA组中的14-3-3η蛋白和抗MCV抗体阳性率分别为68.00%、74.00%,均高于OA组和HC组,单独检测14-3-3η蛋白和抗MCV抗体对RA诊断的灵敏度均远低于抗CCP抗体,但14-3-3η蛋白和抗MCV抗体联合检测,灵敏度和特异性都得到了显著性提高,其敏感性为91.68%,特异性高达99.17%。结论:14-3-3η蛋白和抗MCV抗体联合检测,并结合抗CCP抗体的表达水平,能帮助提高类风湿关节炎的诊断准确率。

Objective:To study the expression of MCV and 14—3—3η levels in serum Rheumatoid arthritis,and to evaluate the clinical significance of the combined detection of RA in the diagnosis of RA.Methods:Fifty patients with rheumatoid arthritis were in the RA group,45 patients with osteoarthritis were in the OA group,and 40 patients in the physical examination were in the HC group.Detection of MCV and 14—3—3η expression in serum by ELISA method,At the same time,it is compared with the commonly used clinical index anti-CCP,and the experimental data is statistically analyzed by using SPSS22.0 software.Results:The positive rates of 14—3—3η and anti—MCV in the RA group were 68.00%and 74.00%.The sensitivity of the detection of protein 14—3—3η and anti—CCP antibody to RA was much lower than that of anti—CCP antibody.However,the combination of 14—3—3η protein and anti—MCV antibody showed significant sensitivity and specificity,with a sensitivity of 91.68%and a specificity of 99.17%.ConClusion:The combined detection of anti—MCV and 14—3—3η has a high diagnostic value for RA.
