基于中医“痰浊”理论探讨多囊卵巢综合征与肥胖

Discussion on Polycystic Ovary Syndrome and Obesity based on the Theory of Phlegm Turbid in Traditional Chinese Medicine

多囊卵巢综合征是临床常见的一种生殖内分泌及代谢性疾病,其主要表现为月经稀发、多毛、肥胖、排卵障碍及不孕等,其中,超重或肥胖者占50%~80%。随着现代人民生活方式的改变及社会压力的增加,肥胖型PCOS的发病率逐年增加。中医多从痰论治肥胖型PCOS,取得一定效果。作者认为“痰”既为多囊卵巢综合征致病因素,亦为其主要病理产物。正如清代沈金鳌云:“痰为诸病之源,怪病皆由痰成”,《丹溪治法心要》中载:“肥白人多痰湿”,说明水湿痰浊是肥胖发生的重要病机之一。两者病因病机相似,互为因果,故作者从“痰浊”理论入手探讨多囊卵巢征与肥胖的相关性,为临床进一步研究及治疗该病提供新的思路。

Polycystic ovary syndrome(PCOS)is a common clinical reproductive endocrine and metabolic disease.Its main manifestations are hypomenorrhea,hirsutism,obesity,ovulation disorders and infertility.Among them,50%-80%of them are overweight or obese.With the change of modern people's lifestyle and the increase of social pressure,the incidence rate of obese PCOS is increasing year by year.Traditional Chinese medicine(TCM)mostly treats obese PCOS from phlegm and achieves certain effect.The author thinks that"phlegm"is not only the pathogenic factor of PCOS,but also the main pathological product.As Shen Jin Ao of the Qing Dynasty said:"phlegm is the source of various diseases,and strange diseases are caused by phlegm".According to"Danxi therapeutic heart essentials",it is stated that"fat white people have more phlegm and dampness",which indicates that water dampness and phlegm turbidity are one of the important pathogenesis of obesity.The etiology and pathogenesis of the two are similar and cause and effect each other.Therefore,the author discusses the correlation between polycystic ovary syndrome and obesity from the"phlegm turbid"theory,so as to provide new ideas for further clinical research and treatment of the disease.
