中药热奄包外敷联合经皮椎体后凸成形术治疗骨质疏松性椎体压缩性骨折疗效观察

Clinical Observation of the Effect of Traditional Chinese Medicine Foment Package Combined with Percutaneous Kyphoplasty in the Treatment of Osteoporotic Vertebral Compression Fractures

目的:观察中药热奄包外敷联合经皮椎体后凸成形术(PKP)对骨质疏松性椎体压缩性骨折(OVCF)患者疼痛的影响。方法:选择80例安徽中医药大学第一附属医院骨一科收治的骨质疏松性椎体压缩性骨折患者,由入院次序依据奇偶数分为对照组(40例)和观察组(40例),对照组单纯采用经皮椎体后凸成形术(PKP),观察组在对照组基础上加用中药热奄包外敷治疗,两组患者性别、年龄、骨密度(BMD)、体重指数(BMI)均无统计学差异。统计两组患者术前及术后视觉模拟评分(VAS)以及Oswestry功能障碍指数(ODI)并评价其疗效。结果:两组患者术后视觉模拟评分(VAS)以及Oswestry功能障碍指数(ODI)均较治疗前显著改善,其中观察组VAS为(2.05±1.41)、ODI为(30.38±13.05),优于对照组VAS为(2.78±1.23)、ODI为(36.80±12.41),两组间指标差别有统计学意义(P<0.05)。观察组优良率82.5%,对照组优良率57.5%,差异有统计学意义(P<0.05)。结论:中药热奄包外敷联合经皮椎体后凸成形术(PKP)较单独经皮椎体后凸成形术(PKP)治疗骨质疏松性椎体压缩性骨折可获得更好的短期疗效,值得在临床推广应用。

Objective:To observe the effect of traditional Chinese medicine hot pack external application combined with percutaneous kyphoplasty(PKP)on the pain of patients with osteoporotic vertebral compression fractures(OVCF).Methods:Eighty patients with osteoporotic vertebral compression fractures who were admitted to the Department of Orthopedics,First Affiliated Hospital of Anhui University of Traditional Chinese Medicine,were divided into control group(40 cases)and observation group(40 cases)according to the odd and even numbers according to the admission order.The control group only used percutaneous kyphoplasty(PKP),and the observation group was treated with traditional Chinese medicine hot packs on the basis of the control group.The gender,age,bone density(BMD)and body mass index(BMI)of the two groups.There was no statistical difference.The preoperative and postoperative visual analogue scale(VAS)and Oswestry dysfunction index(ODI)of the two groups of patients were counted and their efficacy was evaluated.Results:Postoperative visual analogue scale(VAS)and Oswestry dysfunction index(ODI)of the two groups of patients were significantly improved compared to before treatment.Among them,the observation group had VAS(2.05±1.41)and ODI(30.38±13.05),which were better than the control Group VAS was(2.78±1.23),ODI was(36.80±12.41),and the index difference between the two groups was statistically significant(P<0.05).The excellent and good rate of observation group was 82.5%and that of control group was 57.5%,the difference was statistically significant(P<0.05).Conclusion:Traditional Chinese medicine hot pack external application combined with percutaneous kyphoplasty(PKP)can achieve better short-term curative effect than percutaneous kyphoplasty(PKP)alone in the treatment of osteoporotic vertebral compression fractures.It is worthy of clinical application.
