集束化护理对脑卒中吞咽功能障碍患者干预效果的meta分析

Intervention effect of cluster nursing on patients with dysphagia caused by stroke:A Meta-analysis

目的运用循证医学方法评价集束化护理对脑卒中吞咽功能障碍患者的干预效果。方法通过计算机检索The Cochrane Library、PubMed、Void、Web of Science、中国知网、万方数据知识服务平台、维普资讯中文期刊服务平台等数据库中有关集束化护理干预脑卒中吞咽功能障碍患者的随机对照试验,检索时间为建库至2020年1月。运用RevMan5.3软件对纳入文献进行meta分析。结果共纳入随机对照试验12篇,包括1165例患者。集束化护理组患者总有效率、吞咽功能障碍评分、生活能力评定量表评分、生活质量评分、汉密尔顿焦虑量表评分、汉密尔顿抑郁量表评分均明显优于常规护理组,差异均有统计学意义(P<0.05)。结论束化护理对脑卒中吞咽功能障碍患者吞咽功能、生活能力及生活质量的提高具有明显效果,对患者的负性情绪具有良好的改善作用。

Objective To evaluate the intervention effect of cluster nursing on patients with stroke and dysphagia.Methods A computerized randomized controlled trial of clustered nursing interventions in patients with stroke and dysphagia,including The Cochrane Library,PubMed,Void,Web of Science,CNKI,Wangfang Data,and VIP Journal Database(VIP)Trial(RCT),the literature search time is from the establishment of the database to January 2020.After the two reviewers evaluated the quality of the literature,they performed a meta-analysis of the included literature using RevMan5.3 software.Results A total of 10 RCTs were included in this article,including 1165 patients.Results of meta analysis show that compared with the conventional nursing group and the clustered nursing group,the total effectiveness of rehabilitation of dysphagia,dysphagia function score,life ability score,quality of life score,Hamilton anxiety scale score,Hamilton depression scale score.The difference was statistically significant(P<0.05).Conclusion Existing evidence shows that cluster nursing has a significant effect on the improvement of swallowing function,living ability and quality of life in patients with stroke and dysphagia,and it has a good effect on improving patients′negative emotions.
