超声弹性成像在乳腺癌NE新辅助化疗方案效果评估中的应用

Application of ultrasound elastography in the effect evaluation of NE neoadjuvant chemotherapy for breast cancer

目的:探讨超声弹性成像(UE)在乳腺癌NE新辅助化疗方案效果评估中的应用。方法:选取接受NE新辅助化疗方案治疗的乳腺癌患者66例,于化疗后进行二维超声成像(2DUS)、超声造影(CEUS)、UE检查,比较3种成像诊断结果与病理结果的差异。结果:化疗4个疗程后,与病理结果相比,2DUS、CEUS、UE的诊断符合率差异不具统计学意义(P>0.05)。UE的诊断灵敏度、特异度、准确性与2DUS、CEUS比较,差异具有统计学意义;UE评估肿瘤测量值与病理测量值的差值明显小于2DUS、CEUS(P<0.05)。结论:与2DUS、CEUS检查相比,UE在乳腺癌NE新辅助化疗方案效果评估中具有较强的灵敏度与准确性,且时效性强、费用低。

Objective:To explore the application of ultrasound elastography(UE)in the effect evaluation of NE neoadjuvant chemotherapy for breast cancer.Methods:A total of 66 breast cancer patients received NE neoadjuvant chemotherapy were selected.After chemotherapy,two-dimensional ultrasound imaging(2DUS),contrast-enhanced ultrasound(CEUS)and UE were performed to compare the differences between the three kinds of imaging diagnosis results and pathological results.Results:After 4 courses of chemotherapy,the diagnostic coincidence rates of 2DUS,CEUS,and UE ultrasound imaging were not statistically significant compared with the pathological results(P>0.05).The diagnostic sensitivity,specificity and accuracy of UE evaluation showed statistically significant compared with 2DUS and CEUS(P<0.05).The differences between UE-assessed tumor measurement value and pathological measurement value were significantly smaller than those of 2DUS,CEUS(P<0.05).Conclusion:Compared with 2DUS and CEUS examinations,UE has stronger sensitivity and accuracy in the evaluation of the effects of NE neoadjuvant chemotherapy regimens for breast cancer,strong timeliness and low cost。
