髌骨骨性关节炎TKA保留髌骨与否的预后分析

Prognostic Analysis of Patella Preservation or not in TKA of Patella Osteoarthritis

目的探讨髌骨关节骨性关节炎全膝关节置换(TKA)中保留髌骨与否对预后的影响效果。方法选择医院收治的TKA治疗的髌骨关节骨性关节炎患者70例(单膝病变)进行研究,根据术中是否保留髌骨分为观察组与对照组,每组35例,观察组不予以髌骨置换(保留髌骨)、对照组予以髌骨置换(不保留髌骨)。比较两组术前与术后半年膝关节KSS评分、HSS评分、髌骨PS评分、疼痛VAS评分。结果术前两组KSS、HSS、PS、VAS评分比较,差异无统计学意义(P>0.05),术后半年观察组KSS、HSS、PS评分更高(P<0.05),VAS评分更低(P<0.05)。结论髌骨关节骨性关节炎TKA治疗中保留髌骨处理,相比不保留髌骨,可缓解术后疼痛,改善膝关节与髌骨评分。

Objective To investigate the effect of patella preservation on the prognosis of total knee arthroplasty(TKA).Methods 70 patients with osteoarthritis of patella treated by TKA in our hospital were selected for study.According to whether the patella was preserved during operation,they were divided into observation group and control group,35 cases in each group.The observation group did not receive patella replacement(patella preservation),and the control group received patella replacement(patella preservation).KSS,HSS,PS and vas were compared before and half a year after operation.Results There was no significant difference in KSS,HSS,PS and VAS scores between the two groups before operation(P>0.05).The scores of KSS,HSS and PS in the observation group were higher(P<0.05)and lower(P<0.05)half a year after operation.Conclusion Compared with no patella,TKA can relieve postoperative pain and improve the score of knee joint and patella.
