脱细胞基质生物补片修补腹股沟嵌顿疝的临床效果分析

Clinical effect analysis of acellular matrix biological mesh for inguinal incarcerated hernia

目的探讨脱细胞基质生物补片修补腹股沟嵌顿疝的临床效果。方法回顾性分析2016年1月至2018年1月本院收治的50例行Lichtenstein"无张力疝修补术"的腹股沟嵌顿疝患者的临床资料。根据补片材料的不同分为实验组(脱细胞基质生物补片组,n=20)与对照组(聚丙烯补片组,n=30)。比较两组患者疝修补的效果及并发症发生情况。结果两组一般临床资料、住院时间、手术时间、术后住院时间、切口感染、术后慢性疼痛等比较差异无统计学意义,两组患者均无疝复发。对照组异物感发生率为30.0%(9例),明显高于实验组的5.0%(1例),差异有统计学意义(P<0.05);实验组血清肿发生率为25.0%(5例)、住院花费为20 197元,对照组血清肿发生率为6.7%(2例),住院花费为15 495元,两组比较差异均有统计学意义(P<0.05)。结论脱细胞基质生物补片应用于腹股沟嵌顿疝修补安全有效,能够降低患者术后异物感发生率,但治疗费用增加。

Objective To investigate the clinical effect of acellular matrix biopsy in repairing incarcerated inguinal hernia.Methods The clinical data of 50 patients admitted to our hospital from January 2016 to January 2018 with inguinal hernia of Lichtenstein who underwent tension-free hernia repair were retrospectively analyzed.According to the patch material,50 patients were divided into experimental group(decellularized matrix bio-supplement group,n=20),and control group(polypropylene patch group,n=30).The effects of hernia repair and complications were analyzed between two groups.Results There was significant statistical difference in the general clinical data,hospitalization time,operation time,postoperative hospital stay,incision infection,and postoperative chronic pain between two groups.There was no hernia recurrence in both groups.The incidence of foreign body sensation in the control group was 30.0%,which was significantly higher than 5.0%in the experimental group,the difference was statistically significant(P<0.05).The incidence of seroma in the experimental group was 25.0%,and the hospitalization cost was 20,197 yuan,the incidence of seroma in the control group was 6.7%,and the hospitalization cost was 15,495 yuan,the difference between the two groups was statistically significant(P<0.05).Conclusion The acellular matrix biopsy is safe and effective for incarcerated inguinal hernia repair,which can reduce the incidence of postoperative foreign body sensation,but the cost of treatment increases.
