SWI和血清表皮生长因子对轻度认知功能障碍患者诊断价值的评估 被引量:3

摘要 目的探究磁敏感加权成像(SWI)扫描、血清表皮生长因子(EGF)在轻度认知功能障碍(MCI)患者诊断中的价值。方法选取2017年1月至2019年1月于本院神经内科就诊的60~75岁患者150例,依据2018年轻度认知功能障碍诊断与治疗指南,分为正常认知组(n=99)和MCI组(n=51)。MCI组为MCI患者,正常认知组为正常认知患者,记录SWI脑微出血灶(CMBs)数量及部位、测定血清EGF水平、记录MoCA评分。对MCI组受试者随访1年,重新进行诊断,统计转变为AD的患者数。结果 MCI组MoCA评分低于正常认知组,MCI组CMBs数量多于正常认知组,MCI组患者EGF水平低于正常认知组,差异均有统计学意义(P<0.05)。随访1年后,约17%的MCI患者转化为AD。结论 MCI早期颅内CMBs病灶数量增多、血清EGF水平下降,可用于MCI的早期诊断。MCI患者血清EGF水平越低预示其转化为AD的风险更高,可作为转化为AD的预测因子。 Objective To investigate the value of susceptibility weighted imaging(SWI)scanning and serum EGF level in MCI's diagnosis.Methods 150 patients from 60 to 75 years old who admitted to the Department of Neurology of our hospital from January 2017 to January 2019 were selected.According to the 2018 Guidelines for the Diagnosis and Treatment of Young Cognitive Dysfunction,they were divided into the normal cognitive group(n=99)and the MCI group(n=51).We chose MCI patients as MCI group and normal cognitive subjects as control group.All subjects took SWI scanning,serum EGF test and MoCA test.1 years later reestimated the MCI group subjects,count out how many subjects turned to AD.Results The MoCA score in the MCI group was lower than that in the normal cognitive group,CMBs count was more than that in the normal cognitive group,EGF level was lower than that in the normal cognitive group,and the differences are statistically significant(P<0.05).1 years later 17%of MCI subjects turned to AD.Conclusion SWI scanning and serum EGF test are available to diagnose MCI in early phase.Low level of serum EGF can predict MCI subject turn to AD.
