WBC和CRP及PCT在肝硬化患者中的表达及与肝功能的相关性研究

目的探讨白细胞(WBC)和C反应蛋白(CRP)以及降钙素原(PCT)在肝硬化患者体内的表达及与肝功能的相关性。方法选取2017年6月至2019年9月本院收治的102例肝硬化患者作为研究对象,根据肝功能Child-Pugh分级分为A组(Child-Pugh A级)37例,B组(ChildPugh B级)35例,C组(Child-Pugh C级)30例,再根据患者是否发生自发性细菌性腹膜炎(SBP)分成SBP组(n=45)与非SBP组(n=57),均抽血检测WBC、CRP、PCT水平,并经由spearman相关性分析分析WBC、CRP、PCT水平与Child-Pugh分级的相关性。结果经检测,A组WBC、CRP、PCT均显著低于B组、C组,且B组WBC、CRP、PCT显著低于C组,差异均有统计学意义(P<0.05);SBP组WBC、CRP、PCT值均显著高于非SBP组,差异均有统计学意义(P<0.05);肝硬化患者肝功能Child-Pugh分级与WBC、CRP、PCT值均高度相关(r=0.605,r=0.623,r=0.615)。结论肝硬化患者WBC、CRP、PCT与肝功能的相关性显著,且WBC、CRP、PCT越高,表明肝功能损伤程度越高。

Objective To investigate the correlation between the expression of white blood cell(WBC),C-reactive protein(CRP)and calcitonin(PCT)and liver function in patients with cirrhosis.Methods 102 patients with cirrhosis who were admitted to the hospital from June 2017 to September 2019 were included in the study.All patients were divided into 37 patients in Group A(Child-Pugh A),Group B(Child-Pugh B)35 cases,In the group C(Child-Pugh C)30 cases according to the classification of liver function.102 patients were divided into SBP group(n=45)and non-SBP group(n=57)according to whether the patient had spontaneous bacterial peritonitis(SBP).Both blood samples were used to detect WBC,CRP,and PCT levels.The correlation between WBC,CRP,and PCT levels and Child-Pugh classification was analyzed by spearman correlation analysis.Results WBC,CRP,and PCT in Group A were significantly lower than those in Group B and C,and WBC,CRP,and PCT in Group B were significantly lower than those in Group C,and the differences were statistically significant(P<0.05).The values of WBC,CRP,and PCT in SBP group were significantly higher than those in non-SBP group,and the difference was statistically significant(P<0.05).Correlation analysis showed that the liver function Child-Pugh classification in patients with cirrhosis was strongly correlated with WBC,CRP,and PCT values(r=0.605,r=0.623,r=0.615).Conclusion The correlation between WBC,CRP and PCT and liver function is significant in patients with cirrhosis,and the higher the WBC,CRP and PCT,the higher the degree of liver function damage.
