摘要
【目的】探讨miRNA-221-3p表达对冠心病患者的诊断价值。【方法】选择2018年5〜11月本院收治的400例行冠脉造影检查的疑似冠心病(CHD)患者,其中290例经冠脉造影检查诊断为CHD患者(观察组),110例为非CHD患者(对照组)。采用实时定量PCR检测两组研究对象血清miRNA-221-3p表达情况,采用受试者工作特征曲线分析miRNA-221-3p诊断冠心病的诊断价值。【结果】两组研究对象在性别、年龄、白细胞计数、血尿酸水平、肌酐、空腹血糖方面比较,差异无统计学意义(P>0.05);观察组高血压、糖尿病、吸烟所占比例及BMI、TG、TC、LDL-C、miRNA-221-3p表达水平显著高于对照组,HDL-C显著低于对照组,差异有统计学意义(P<0.05)。ROC曲线分析显示,miRNA-221-3p诊断CHD最佳临界点为3.7,当miRNA-221-3p为3.7时,miR-NA-221-3p预测CHD发病的灵敏度为66.9%,特异度为70.9%;AUC曲线下面积为0.71。【结论】miRAN-221-3p可作为诊断冠心病的潜在生物标志物。
【Objective】To investigate the diagnostic value of the expression of microRNA221-3p in patients with coronary heart disease.【Methods】From May to November 2018,400 suspected CHD patients were selected,290 of whom were diagnosed as CHD by coronary angiography(observation group)and HO as non CHD patients(control group).The expression of miRNA-221-3p in serum of the two groups was detected by real-time quantitative PCR,and the diagnostic value of miRNA-221-3p in diagnosing coronary heart disease was analyzed by working characteristic curve of the subjects.【Results】There was no significant difference in gender,age,leukocyte count,serum uric acid level,creatinine and fasting blood glucose between the two groups(P>0.05);the proportion of hypertension,diabetes,smoking and the expression level of BMI,TG,TC,LDL-C and miRNA-221-3p in the observation group were significantly higher than those in the control group,while HDL-C was significantly lower than that in the control group(P<0.05).ROC curve analysis showed that the best critical point of miRNA-221-3p for the diagnosis of CHD was 3.7.When miRNA-221-3p was 3.7,the sensitivity and specificity of miRNA-221-3p for the prediction of CHD were 66.9%and 70.9%respectively,and the area under AUC curve was 0.71.【Conclusion】miRNA-221-3p can be used as a potential biomarker for the diagnosis of coronary heart disease.
