自拟消滞通络饮治疗气滞血瘀型0级糖尿病足对患者踝肱指数及生活质量的影响

Effect of Using Self-made Xiaozhi Tongluo Drink in the Treatment of Qi-stagnation and Blood Stasison Type Grade 0 Diabetic Foot and Its Influence on Ankle Brachial Index and Quality of Life

目的:观察自拟消滞通络饮治疗气滞血瘀型0级糖尿病足对患者踝肱指数(ABI)及生活质量的影响。方法:将2017年10月~2019年10月期间78例0级糖尿病足患者采用系统随机化法分为观察组、对照组各39例,两组患者均予以降糖、降压等基础治疗,对照组在基础治疗的基础上加服甲钴胺片,观察组在基础治疗的基础上加服自拟消滞通络饮。比较两组患者治疗8周后临床疗效,以及治疗前后血液流变学指标(血浆黏度、全血粘度高切、红细胞沉降率)、ABI、生活质量[糖尿病治疗影响量表(DIMS)]评分变化。结果:治疗8周后,观察组患者临床总有效率明显高于对照组(P<0.05);两组患者血浆黏度、全血粘度高切、红细胞沉降率均较治疗前降低,且观察组低于对照组(P均<0.05);两组患者ABI均较治疗前升高,且观察组高于对照组(P均<0.05);两组患者DIMS一般症状、糖尿病相关性症状、糖尿病相关精神状态、患者幸福度、社会责任达成各维度评分均较治疗前升高,且观察组高于对照组(P均<0.05)。结论:自拟消滞通络饮治疗气滞血瘀型0级糖尿病足疗效确切,有助于改善患肢功能,提高患者生活质量。

Objective:To study the effect of Self-made Xiaozhi Tongluo Drink on ankle brachial index(ABI)and quality of life for grade 0 diabetic foot with Qi-stagnation and blood-stasis.Methods:From October 2017 to October 2019,78 patients with grade 0 diabetic foot were divided into observation group and controlled group by systematical randomization,with 39 cases in each group.Patients in both groups were given basic treatments such as hypoglycemic therapy and antihypertensive therapy,and the controlled group was additionally given Mecobalamin Tablets on the basis of basic treatments,and the observation group was additionally given Self-made Xiaozhi Tongluo Drink on the basis of basic treatments.The clinical efficacy after 8 weeks of treatment,and hemorheology indexes(plasma viscosity,whole blood high-shear viscosity,erythrocyte sedimentation rate),ABI and quality of life[diabetes treatment impact scale(DIMS)]before and after treatment were compared between the two groups.Results:After 8 weeks of treatment,the total clinical effective rate in the observation group was significantly higher than that in the controlled group(P<0.05).The plasma viscosity,whole blood high-shear viscosity and erythrocyte sedimentation rate in the two groups were lower than those before treatment,and the indexes in the observation group were lower than those in the controlled group(all P<0.05).The ABI in both groups was higher than that before treatment,and the ABI in the observation group was higher than that in the controlled group(all P<0.05).The scores of dimensions of general symptoms,diabetes-related symptoms,diabetes-related mental status,patients’happiness and social responsibility and achievement of DIMS in the two groups were higher than those before treatment,and the scores in the observation group were higher than those in the controlled group(all P<0.05).Conclusion:Self-made Xiaozhi Tongluo Drink has a definite efficacy in the treatment of grade 0 diabetic foot with Qi-stagnation and blood stasis,and it can help improve the function of affected limbs and enhance the quality of life of patients.
