摘要
目的探讨加速康复外科(enhanced recovery after surgery,ERAS)在军事训练伤致前交叉韧带(anterior cruciate ligament,ACL)断裂围手术期护理中的应用。方法随机选取68例军事训练伤致ACL断裂行关节镜下韧带重建的患者,按照围手术期护理方法不同分为ERAS组和传统治疗组,每组34例。比较2组患者术后不同时间疼痛评分、术后6个月的Lysholm膝关节评分、术后48 h的日常生活能力评定及护理满意度。结果ERAS组术后6、12、24、48 h VAS评分明显低于传统康复组,差异均有统计学意义(P<0.05),与传统组比较,ERAS组术后6个月Lysholm膝关节评分和术后48 h的ADL日常生活能力评定均增高,差异有统计学意义(P<0.001),ERAS组护理满意度也明显高于传统治疗组。结论军事训练伤致ACL断裂围手术期护理应用ERAS理念可减轻病人术后疼痛、提高患者的术后生活质量、促进膝关节功能恢复、提高护理满意度。
Objective To explore the applicability of ERAS to perioperative nursing of patients with anterior cruciate ligament rupture caused by military training.Methods Sixty-eight patients undergoing arthroscopic ACL reconstruction due to military training injuries were randomly selected.According to approaches to perioperative nursing,these patients were divided into the enhanced recovery group(n=34)and the conventional recovery group(n=34).The degree of pain,knee Lysholm score,daily living ability assessment and levels of satisfaction with nursing were compared between the two groups.Results The VAS scores in the ERAS group were significantly lower than those in the conventional recovery group at 6,12,24,and 48 hours after surgery(P<0.05).Compared with the conventional recovery group,the Lysholm knee score and ADL daily life ability in the ERAS group were increased,and the difference was statistically significant(P<0.001).The ERAS group was much more satisfied with nursing than the conventional recovery group.Conclusion ERAS concepts,when applied to the perioperative nursing of ACL caused by military training injury,can reduce postoperative pain,improve postoperative quality of life,promote knee function recovery,and lead to more satisfaction with nursing.
