补气升阳方治疗脾肺气虚型慢性肾小球肾炎疗效观察

Observation on curative effect of Buqi Shengyang Decoction on chronic glomerulonephritis of spleen and lung Qi deficiency

目的观察补气升阳方治疗脾肺气虚型慢性肾小球肾炎的疗效。方法将2018年1月—2019年5月上海市杨浦区控江医院诊治的82例脾肺气虚型慢性肾小球肾炎患者按照随机数字表法分为2组,对照组41例予西医常规治疗,治疗组41例在对照组治疗基础上予补气升阳方治疗,2组均连续治疗12周。观察2组治疗前后中医症状积分、24 h尿蛋白定量、血白蛋白、肾功能指标变化,比较2组疗效及治疗安全性。结果2组治疗后面浮肢肿、腰膝酸痛、疲乏无力、食少纳呆、夜尿频繁症状积分均较治疗前明显降低(P均<0.05),且治疗组各项评分均明显低于对照组(P均<0.05);2组治疗后24 h尿蛋白定量及血肌酐(SCr)、尿素氮(BUN)水平均较治疗前明显降低(P均<0.05),血白蛋白水平及肾小球滤过率(GFR)均较治疗前明显升高(P均<0.05),且治疗组各指标改善情况均明显优于对照组(P均<0.05);治疗组总有效率为80.5%(33/41),对照组为61.0%(25/41),2组比较差异有统计学意义(P<0.05);2组患者在治疗期间均无明显不良反应发生,肝功能和血常规均正常。结论补气升阳方联合西医常规治疗能够有效缓解脾肺气虚型慢性肾小球肾炎患者临床症状,降低24 h尿蛋白定量,提高血白蛋白水平,改善肾功能,有助于延缓疾病进展。

Objective It is to observe the curative effect of Buqi Shengyang Decoction in the treatment of chronic glomerulonephritis with deficiency of spleen and lung Qi.Methods 82 patients with chronic glomerulonephritis of type of spleen lung Qi deficiency diagnosed and treated in Kongjiang Hospital from January 2018 to May 2019 were divided into 2 groups according to a random number table.41 patients in the control group were treated with conventional western medicine,41 cases in the treatment group were treated with Buqi Shengyang Decoction on the basis of the treatment in the control group.Both groups were treated for 12 consecutive weeks.The changes of TCM symptom scores,24-hour urine protein quantification,serum albumin,and renal function indexes before and after treatment were observed in the two groups,and the efficacy and safety of the two groups were compared.Results The scores of symptoms of edema,waist and knee pain,fatigue,lack of appetite,and frequent nocturia after treatment in the two groups were significantly lower than those before treatment(P<0.05),and the scores of each item of symptom of the treatment group were significantly lower than those of the control Group(all P<0.05);24 h urine protein quantitative and the levels of serum creatinine(SCr)and urea nitrogen(BUN)after treatment in the two groups were significantly lower while serum albumin level and glomerulus filtration rate(GFR)was significantly higher than those before treatment(all P<0.05),and the improvement of each index in the treatment group was significantly better than that of the control group(all P<0.05);the total effective rate of the treatment group was 80.5%(33/41),the control group was 61.0%(25/41),the difference between the two groups was statistically significant(P<0.05);there were no obvious adverse reactions in the two groups during the treatment period,and the liver function and blood routine were normal.Conclusion Buqi Shengyang Decoction combined with conventional western medicine can effectively relieve the clinical symptoms,reduce 24h urine protein quantification,increase blood albumin level,improve renal function,and help delay disease progression in the patients with chronic glomerulonephritis with deficiency of spleen and lung Qi.
