盆腔脓肿56例临床分析

The Clinical Analysis of 56 Cases of Pelvic Abscess

目的探讨盆腔脓肿患者的最佳治疗方法及手术方式。方法回顾性分析56例盆腔脓肿患者的临床资料,其中22例接受抗菌药物保守治疗(非手术组),34例接受抗菌药物联合手术治疗(手术组18例接受腹腔镜手术,16例接受开腹手术)。比较两组患者的临床治疗有效率,分析手术组中腹腔镜手术组和开腹手术组的手术时间、术中出血量、住院时间、并发症发生率。结果非手术组和手术组的临床有效率分别为81.82%和100%,两组比较,差别具有统计学意义(P<0.05);与开腹手术组比较,腹腔镜手术组的手术时间短、术中出血量少、住院时间短,差别具有统计学意义(P<0.05)。结论抗菌药物联合腹腔镜手术治疗盆腔脓肿具有创伤小、恢复快、效果佳等优势,可有效提高疗效。

Objective To explore the best method of treatment and operation for patients with pelvic abscess.Methods Review the diagnosis,treatment and prognosis of 56 patients with pelvic abscess.Compare and analyze them.22 patients received conservative treatment with antibiotics(non-operation group).34 patients received combined surgical treatment with antibiotics(operation group:18 patients group laparoscopic surgery and 16 patients received open surgery).Compared the clinical efficacies of the two groups.The operation time,intraoperative bleeding volume,hospitalization time and complication rate of laparoscopic surgery group and open surgery group were compared and analyzed.Results The clinical effective rate was 81.82%in the nonoperative group and 100%in the operative group,which was statistically significant(P<0.05).The laparoscopic group had shorter operation time,less intraoperative bleeding and shorter hospital stay than the open group(P<0.05).Conclusion The combination of antibiotics and laparoscopic surgery for pelvic abscess has the advantages of small trauma,fast recovery and good effect,which can effectively improve the efficacy of patients.
