血清心肌肌钙蛋白Ⅰ和血浆B型钠尿肽联合检测在心力衰竭中的应用价值分析 被引量:4

摘要 目的探讨血清心肌肌钙蛋白I(cTnI)和血浆B型钠尿肽(BNP)联合检测在心力衰竭中的应用价值。方法收集2019年6月~2020年4月106例心力衰竭患者作为观察组,抽取同期因肺部疾病住院无心力衰竭的患者60例作为对照组。分别于入院后24h内抽血检测两组患者的血清cTnI、血浆BNP,并采用飞利浦EPIQ 7C Evolution3.0专业心血管超声系统检测患者的左室射血分数(LVEF)。结果(1)观察组血cTnI、BNP均高于对照组,LVEF低于对照组(P<0.05);(2)观察组随着NYHA分级升高,血cTnI、BNP逐渐增高,LVEF逐渐下降,不同NYHA分级之间cTnI、BNP及LVEF比较,差异均有统计学意义(P<0.05);(3)观察组不同LVEF分组患者血cTnI、BNP不同,LVEF越低,血cTnI、BNP越高,NYHA分级越重(P<0.01)。结论cTnI和BNP是心力衰竭重要的诊断指标,能反映心肌程度和判断病情程度,为患者的早期诊断、病情判断、治疗指导、疗效评价及预后评估提供了客观、有效的指标。 Objective To investigate the application value of combined detection of serum cardiac troponin I(cTnI)and plasma B-type natriuretic peptide(BNP)in heart failure.Methods Patients with heart failure from June 2019 to April 2020 were collected as the observation group(n=106),and those without heart failure who were hospitalized due to lung diseases in the same period were selected as the control group(n=60).Serum cTnI and plasma BNP were measured within 24 hours after admission,and left ventricular ejection fraction(LVEF)was measured by Philips EPIQ 7C Evolution 3.0 professional cardiovascular ultrasound system.Results(1)The blood cTnI and BNP in the observation group were higher than those in the control group,and LVEF was lower than that in the control group(P<0.05).(2)With the increase of NYHA grade,the blood cTnI and BNP gradually increased and LVEF gradually decreased in the observation group,and there were statistically significant differences in cTnI,BNP and LVEF among different NYHA grades(P<0.05).(3)The levels of cTnI and BNP were different in patients with different LVEF grades in the observation group.The lower LVEF was,the higher the levels of cTnI and BNP were,and the higher NYHA grade was(P<0.01).Conclusion cTnI and BNP are important diagnostic indexes of heart failure,which can reflect the myocardium degree and judge disease severity,and can be served as objective and effective indexes for early diagnosis,disease severity judgment,treatment guidance,curative efficacy evaluation and prognosis evaluation of patients.
