榆黄抗菌散联合硝酸咪康唑乳膏治疗足癣的效果及其对浅部真菌的影响

Effect of Yuhuang antibacterial powder combined with miconazole nitrate cream in the treatment of tinea pedis and its influence on superficial fungi

目的探讨榆黄抗菌散联合硝酸咪康唑乳膏治疗足癣的效果及其对浅部真菌的影响。方法选取2017年4月~2019年8月茂名市中医院收治的92例足癣患者作为研究对象,根据随机数字表法分为观察组和对照组,每组各46例。对照组采用硝酸咪康唑乳膏治疗,观察组在对照组治疗基础上加用榆黄抗菌散,治疗2个月,观察两组中医症候积分、临床疗效、浅部真菌抑制情况及生活质量。采用微量稀释法测定榆黄抗菌散对红色毛癣菌、须癣毛癣菌和白念珠菌的最低抑菌浓度。结果观察组治疗后中医证候积分低于对照组,总有效率(95.65%,44/46)明显高于对照组(82.61%,38/46),治疗后生理评分、心理评分、依从性评分高于对照组,差异均有统计学意义(P<0.05);观察组浅部真菌抑制率高于对照组,差异有统计学意义(P<0.05);榆黄抗菌散对白念珠菌、须癣毛癣菌、红色毛癣菌的最低抑菌浓度分别为0.12500g/mL、0.50000g/mL、0.25000g/mL,抑菌浓度越小则抑菌强度越大。结论榆黄抗菌散联合硝酸咪康唑乳膏治疗可提高足癣患者的临床效果和生活质量,还能有效抑制浅部真菌。

Objective To explore the effect of Yuhuang antibacterial powder combined with miconazole nitrate cream in the treatment of tinea pedis and its influence on superficial fungi.Methods A total of 92 patients with tinea pedis admitted to Maoming Hospital of Traditional Chinese Medicine from April 2017 to August 2019 were selected as the research objects,and were divided into observation group and control group according to the random number table method,with 46 cases in each group.The control group was treated with miconazole nitrate cream,while the observation group was treated with Yuhuang antifungal powder on the basis of the control group,treated for 2 months.The TCM symptom scores,clinical efficacy,superficial fungal inhibition and quality of life(QoL)of the two groups were observed.The minimum inhibitory concentration of Yuhuang antifungal powder against Trichophyton rubrum,Trichophyton mentagrophytes and Candida albicans was determined by microdilution method.Results After treatment,the TCM syndrome scores of the observation group were lower than those of the control group,while the total effective rate was 95.65%(44/46),obviously higher than 82.61%of the control group(38/46),and the physiological scores,psychological scores and compliance scores were higher than those of the control group,the differences were statistically significant(P<0.05).The superficial fungal inhibition rate in the observation group was higher than that in the control group,the difference was statistically significant(P<0.05).The minimum inhibitory concentration of Yuhuang antifungal powder against Candida albicans,Trichophyton mentagrophytes and Trichophyton rubrum was 0.12500g/mL,0.50000g/mL and 0.25000g/mL respectively.The lower the inhibitory concentration,the higher the inhibitory intensity.Conclusion Yuhuang antibacterial powder combined with miconazole nitrate cream can improve the clinical effect and quality of life of patients with tinea pedis,and can effectively inhibit superficial fungi.
