单孔腹腔镜治疗小儿非交通性鞘膜积液的临床研究

Clinical research on single-port laparoscopy in the treatment of children with non-communicating hydrocele

目的研究单孔腹腔镜治疗小儿非交通性鞘膜积液的临床效果。方法将我院2017年9月~2019年6月收治的86例非交通性鞘膜积液小儿作为研究对象,按照自愿原则,将其分为A组54例和B组32例,前者采用单孔腹腔镜鞘状突高位结扎术治疗,后者采用传统鞘状突高位结扎术治疗,比较两组手术切口长度、手术时间、术中出血量、住院时间、住院费用、术后切口疼痛程度、术后并发症等指标。结果A组手术切口长度、手术时间、住院时间均短于B组(P<0.05),术中出血量少于B组(P<0.05),住院费用高于B组(P<0.05);A组术后6、12和24h切口疼痛程度均轻于B组(P<0.05);A组术后并发症总发生率低于B组(P<0.05)。结论单孔腹腔镜治疗小儿非交通性鞘膜积液的临床效果较好,能够减小创口,减轻患儿术后疼痛,减少并发症发生。

Objective To research the clinical efficacy of single-port laparoscopy in the treatment of children with non-communicating hydrocele.Methods A total of 86 children with non-communicating hydrocele admitted to our hospital from September 2017 to June 2019 were selected and divided into group A(n=54)and group B(n=32)according to the principle of voluntariness.The former was treated with single-port laparoscopic high ligation of sheath process,while the latter was treated with conventional high ligation of sheath process.Indexes such as the length of incision,operation duration,intraoperative hemorrhage volume,hospitalization time,hospitalization expenses,postoperative incision pain and postoperative complications were compared between the two groups.Results The length of incision,operation duration and hospitalization time in group A were all shorter than those in group B(P<0.05),the intraoperative hemorrhage volume was less than that in group B(P<0.05),and the hospitalization expense was higher than that in group B(P<0.05).The degree of incision pain in group A was lighter than those in group B at 6h,12h and 24h after operation(P<0.05).The total incidence of postoperative complications in group A was lower than that in group B(P<0.05).Conclusion Single-port laparoscopy is effective in treating children with non-communicating hydrocele,which can reduce the wound,relieve the postoperative pain in children and decrease the incidences of complications.
