临床护理路径联合循证护理在腹腔镜胆囊切除术中的应用价值

Application value of clinical nursing pathway combined with evidence-based nursing in laparoscopic cholecystectomy

目的观察临床护理路径(CNP)联合循证护理(EBN)在腹腔镜胆囊切除术(LC)中的应用价值。方法选择本院普外科2017年2月~2019年10月住院行LC治疗的患者76例作为研究对象,根据随机数字表法分为两组,每组各38例,对照组采用常规护理措施予以干预,观察组在对照组护理基础上予以CNP联合EBN干预。比较两组患者术后恶心呕吐、胆漏、腹腔出血、呼吸道感染及排尿困难等并发症发生率,比较两组患者术后住院时间及住院总费用,采用自制调查表评估两组患者护理满意度。结果观察组患者术后并发症总发生率(13.16%)低于对照组(34.21%),差异有统计学意义(P<0.05);观察组患者术后住院时间短于对照组,住院总费用少于对照组,差异有统计学意义(P<0.05);观察组患者护理总满意度(94.74%)高于对照组(73.68%),差异有统计学意义(P<0.05)。结论LC患者采用CNP联合EBN干预可明显降低术后并发症总发生率,减少住院总费用和缩短住院时间,同时还可提高患者护理总满意度,具有较好的临床应用价值。

Objective To observe the application value of clinical nursing pathway(CNP)combined with evidence-based nursing(EBN)in laparoscopic cholecystectomy(LC).Methods A total of 76 patients treated with LC operation in the department of general surgery and admitted to our hospital from February 2017 to October 2019 were selected as the research objects and divided into the control group(n=38)and the observation group(n=38)according to the random number table method.The control group was intervened by conventional nursing measures,while the observation group was intervened by CNP combined with EBN on the basis of nursing in the control group.The incidence of postoperative complications such as nausea and vomiting,bile leakage,abdominal hemorrhage,respiratory tract infection and dysuria were compared between the two groups.The hospitalization time and total hospitalization expenses were compared between the two groups.Meanwhile,the self-made questionnaire was used to evaluate the nursing satisfactory degree of the two groups.Results The total incidence of postoperative complications in the observation group was 13.16%,which was lower than 34.21%in the control group,and the difference between the two groups was statistically significant(P<0.05).The postoperative hospitalization time of the observation group was shorter than that of the control group,the total hospitalization expense was less than that of the control group,and the differences between the two groups were statistically significant(P<0.05).The total nursing satisfactory degree of patients in the observation group was 94.74%,which was higher than 73.68%in the control group,and the difference between the two groups was statistically significant(P<0.05).Conclusion The intervention of CNP combined with EBN in LC patients can significantly reduce the total incidence of postoperative complications,decrease the total hospitalization expenses,shorten the hospitalization time,and at the same time,it can also improve the total nursing satisfactory degree of patients,which is of good clinical application value.
