经腹腹膜前腔镜腹股沟疝修补术(TAPP)治疗的临床效果研究

Clinical Study of Transabdominal Preperitoneal Endoscopic Inguinal Hernia Repair(TAPP)

目的分析经腹腹膜前腔镜腹股沟疝修补术(TAPP)治疗的临床效果研究。方法选取江苏省溧阳市中医医院2015年1月至2019年12月接受治疗的120例腹股沟疝患者为研究对象,按照随机数字表法分成对照组(n=60)与观察组(n=60)。观察组采用经腹腹膜前腔镜腹股沟疝修补术(TAPP)治疗,观察组采用开放式手术治疗,将两组的治疗结果进行对比。结果观察组术后肠鸣音恢复、肛门排气、自主活动、术后痛感持续及住院等的时间均较对照组短(P<0.05)。对比术后8 h、术后12 h、术后24 h和术后48 h的视觉模拟疼痛(VAS)评分,对照组均高于观察组(P<0.05)。观察组发生阴囊肿胀、精索静脉曲张、腹胀、睾丸坠胀感、切口脂肪液化、腹股沟区疼痛及尿潴留的总发生率低于对照组(P<0.05)。结论对于腹股沟疝患者来说,采用经腹腹膜前腔镜腹股沟疝修补术进行治疗有利于加快患者术后康复,减轻术后疼痛,降低并发症的发生概率,改善预后效果,适合临床参考借鉴。

Objective To analyze the experience of transabdominal anterior peritoneal endoscopic inguinal hernia repair(TAPP).Methods Totally 120 patients with inguinal hernia who received treatment from January 2015 to December 2019 in Liyang Hospital of Traditional Chinese Medicine,were selected as study subjects.According to the random number table,they were divided into control group(n=60)and observation group(n=60).The observation group was treated with transabdominal anterior peritoneal endoscopic inguinal hernia repair(TAPP),and the observation group was treated with open surgery.The treatment results of the two groups were compared.Results The duration of bowel sound recovery,anal exhaust,voluntary activity,postoperative pain duration and hospitalization in the observation group were shorter than those in the control group(P<0.05).VAS scores 8 h,12 h,24 h and 48 h after surgery were compared in the control group,which were all higher than those in the observation group(P<0.05).The total incidence of scrotal swelling,varicocele,abdominal distension,testicular bulge,incision fat liquefaction,inguinal pain and urinary retention in the observation group was lower than that in the control group(P<0.05).Conclusion For patients with inguinal hernia,the use of abdominal preperitoneal endoscopic inguinal hernia repair is conducive to the acceleration of postoperative rehabilitation,reduce postoperative pain,reduce the incidence of complications,improve the prognosis,which is suitable for clinical reference.
