男性泌尿生殖系统非霍奇金淋巴瘤的临床特征及预后分析

摘要 目的研究男性泌尿生殖系统淋巴瘤患者的临床病理特征,并对其预后进行分析。方法回顾性分析甘肃省兰州市西固区人民医院2016年7月至2017年6月收治的9例男性泌尿生殖系统淋巴瘤患者的临床资料,并对其进行探究分析。结果9例患者有6例患者恢复较为顺利,没有出现相关并发症的情况,经过治疗后均被诊断为非霍奇金淋巴瘤。根据淋巴瘤Ann Arbor分期标准,确诊为ⅠE~ⅡE期有4例,ⅢE~ⅣE期有5例。根据IPI评分显示,2分以下的有6例,3分以上的有3例。对上述9例患者的随访时间为6至24个月,有7例患者存活,有2例患者经过化疗后完全缓解,另有1例患者经过化疗后部分缓解;3例拒绝化疗的患者,没有发现肿瘤复发。1例患者在接受化疗过程当中,因出现细菌真菌混合感染性肺炎,所以停止化疗。2例死亡患者,分别于确诊6个月以及17个月后因肿瘤进展而死亡。结论男性泌尿生殖系统淋巴瘤容易误诊,临床上在诊断时要严格按照组织病理学这一诊断的金标准,对非霍奇金淋巴瘤的治疗手段应以R-CHOP化疗方案为主。 Objective To study the clinicopathological features and prognosis of male patients with urogenital lymphoma.Methods The clinical data of 9 male patients with urogenital lymphoma admitted to Xigu District People's Hospital of Lanzhou City from July 2016 to June 2017 were retrospectively analyzed.Results The above 6 patients recovered smoothly without any complications.All of them were diagnosed as NHL after treatment.According to Ann Arbor staging criteria,4 cases were diagnosed as stageⅠ-ⅡE and 5 cases were stageⅢE-ⅣE.According to the IPI score,there were 6 cases with less than 2 points and 3 cases with more than 3 points.During the follow-up period of 6 to 24 months,7 patients survived.Among the 3 patients who completed chemotherapy,2 patients achieved complete remission,1 patient achieved partial remission,and 3 patients who refused chemotherapy did not find tumor recurrence.1 patient stopped chemotherapy because of bacterial and fungal infection pneumonia during chemotherapy.2 patients died 6 months after diagnosis and 17 months after diagnosis.The cause of death was the further development of tumor.Conclusion Male genitourinary system lymphoma is very easy to be misdiagnosed.Clinical diagnosis should be made in strict accordance with the gold standard of histopathology.The treatment of non Hodgkin's lymphoma should be based on R-CHOP chemotherapy.
