Relationship Between Prognostic Nutritional Index and Prognosis of Breast Cancer Patients

目的研究预后营养指数(PNI)在乳腺癌患者预后的预测价值。方法回顾性分析乐山市人民医院2016年6月至2018年1月收治的150例女性乳腺癌患者的临床资料,其中三阴性乳腺癌患者24例。采用χ2检验分析PNI与乳腺癌患者临床特征的关系,Cox回归分析影响乳腺癌患者预后的危险因素。结果不同PNI患者年龄、淋巴结转移、雌激素受体(ER)、孕激素受体(PR)、人类表皮生长因子受体-2(HER-2)状态对比差异有统计学意义(P<0.05);T分期、增殖细胞核抗原(Ki-67)组间数据对比差异无统计学意义(P>0.05)。单因素分析结果显示,不同PNI的乳腺癌患者的无病生存期(DFS)比较,差异有统计学意义(HR=2.897,95%CI=1.031~8.138,P=0.044);多因素分析结果显示,高PNI是乳腺癌患者预后的独立危险因素(HR=3.019,95%CI=1.074~8.484,P=0.036)。但是,PNI在三阴性乳腺癌患者无病生存期(DFS)中,差异无统计学意义(P=0.606)。结论术前PNI是乳腺癌患者术后复发转移的可靠预测指标。

Objective To investigate the value of preoperative prognostic nutritional index(PNI)in predicting survival of breast cancer patients.Methods Retrospective analysis was performed on 150 female breast cancer patients admitted to Leshan People's Hospital from June 2016 to January 2018 as the study subjects,including 24 patients with triple-negative breast cancer.The relationship between PNI and clinical characteristics of breast cancer patients was analyzed by chi-square test,and the risk factors affecting the prognosis of breast cancer patients were analyzed by Cox regression analysis.Results Age,lymph node metastasis,status of estrogen receptor(ER),progesterone receptor(PR)and human epidermal growth factor receptor-2(HER-2)were significantly different in different PNI patients(P<0.05),while data of T stage and proliferative cell antigen(Ki-67)were not significantly different(P>0.05).Univariate analysis showed that the difference in disease-free survival(DFS)among breast cancer patients with different PNI was statistically significant(HR=2.897,95%CI=1.031~8.138,P=0.044).Multivariate analysis showed that high PNI was an independent risk factor for prognosis of breast cancer patients(HR=3.019,95%CI=1.074~8.484,P=0.036).However,the difference of PNI in DFS of patients with triple negative breast cancer was not statistically significant(P=0.606).Conclusion The preoperative PNI may be a reliable predictor of DFS of patients with breast cancer.
