摘要
目的调查杭州市2017年1起医院内水痘聚集性疫情的发生原因。方法对该医院水痘患者进行个案调查,采集患者疱疹液,进行水痘-带状疱疹病毒(VZV)核酸检测、基因分型和毒株鉴定,描述性分析疫情特征和发生原因。结果2017年8月19日-9月18日该医院血液病科共发生5例水痘病例,包括3名住院患者和2名护士。3名住院患者的疱疹液标本VZV核酸检测呈阳性;SNP和RFLP分析显示,阳性病毒株均属于clade2基因型VZV野毒株(Pst I^+Bgl I^+Sma I^-)。结论该起水痘聚集性疫情由VZV医院感染引起,未严格执行隔离和防护措施是发生疫情的主要原因。
Objective To determine the causes of a varicella cluster in a hospital of Hangzhou city in 2017.Methods We conducted an epidemiological investigation of varicella cases and collected vesicle fluid samples for varicellazoster virus(VZV)DNA detection,genotyping,and strain identification.We analyzed characteristics and causes of the cluster by descriptive epidemiological methods.Results A total of 5 individuals-3 inpatients and 2 nurses–were diagnosed with varicella in the hematology department of the hospital from August 19 to September 18,2017.The 3 inpatient cases were positive to VZV DNA.SNP and RFLP analysis showed that the strains were wild VZV strains(Pst I+Bgl I+Sma I-)and belonged to clade 2 genotype.Conclusion The varicella cluster was caused by a nosocomial VZV infection,with main causes being faulty contact isolation and lack of individual protection.
