中文版炎症性肠病残疾指数的信效度研究

Study on reliability and validity of Chinese version of inflammatory bowel disease disability index

目的:翻译验证中文版炎症性肠病残疾指数(IBD-DI),并用于评价我国炎症性肠病(IBD)患者的残疾程度。方法:在获得IBD-DI原作者Laurent Peyrin-Biroulet的同意后,遵循《自我报告量表跨文化适应过程指南》翻译出中文版IBD-DI。对2018年10-12月在苏州大学附属第二医院随访的IBD患者发放调查问卷。对IBD-DI行效度及信度分析,采用因子分析和相关分析评价结构效度,采用组内相关系数(ICC)评价复测信度,采用Spearman-Brown折半信度系数分析折半信度,采用Cronbach′sα系数分析内部一致性信度。采用IBD-DI评估研究人群的残疾程度。结果:共有170例IBD患者参与这项研究,其中溃疡性结肠炎(UC)63例,克罗恩病(CD)107例。因子分析将中文版IBD-DI的最终版本减少到14个问题,分别对应3个主成分。IBD-DI和炎症性肠病问卷(inflammatory bowel disease questionnaire,IBDQ)之间有高度负相关关系(IBD:r=-0.830,CD:r=-0.844,UC:r=-0.808,均P<0.01)。IBD-DI(复测)和IBD-DI(首次)的组内相关系数(ICC)为0.81(95%CI:0.639~0.905)。Spearman-Brown折半信度系数值为0.786,Cronbach′α系数为0.866,说明数据信度质量好。本组患者IBD-DI得分范围为0~66.07,中位数19.64(12.05,30.36)。结论:中文版IBD-DI是一种可靠、有效的评估中国IBD患者残疾程度的工具。

Objective To translate and verify the Chinese version of the inflammatory bowel disease(IBD)disability index(IBD-DI),and to apply the IBD-DI to evaluate the degree of disability in patients with IBD in China.Methods After obtaining the agreement of Laurent Peyrin-Biroulet,the author of IBD-DI,the Chinese version of IBD-DI was translated under the guidelines for the process of cross-cultural adaptation of self-report measures.Questionnaires were conducted to IBD patients who were followed up in the Second Affiliated Hospital of Soochow University from October to December 2018.The validity and reliability of IBD-DI were analyzed.Factor analysis and correlation analysis were used to evaluate the structural validity.The intraclass correlation coefficient(ICC)was used to evaluate retest reliability.The Spearman-Brown split half reliability coefficient was used to analyze split half reliability and Cronbach′sαcoefficient was used to analyze internal consistency of reliability.IBD-DI was used to evaluate the disability degree of the participants.Results A total of 170 IBD patients participated in the study,including 63 with ulcerative colitis(UC)and 107 with Crohn′s disease(CD).Factor analysis reduced the final version of the Chinese version of IBD-DI to 14 questions,corresponding to 3 main components.There was a highly negative correlation between IBD-DI and inflammatory bowel disease questionnaire(IBDQ)(IBD:r=-0.830,CD:r=-0.844,UC:r=-0.808,all P<0.01).ICC of IBD-DI(retest)and IBD-DI(first)was 0.81(95%CI:0.639-0.905).Spearman-Brown split half reliability coefficient was 0.786,and Cronbach′αcoefficient was 0.866,which indicated the data reliability and quality was good.IBD-DI scores ranged from 0 to 66.07,with an median score of 19.64(12.05,30.36).Conclusion The Chinese version of IBD-DI is a reliable and effective tool for assessing disability of IBD patients in China.
