2009-2018年虹口区手足口病流行病学及病原学特征分析

Epidemiological and etiological characteristics of hand-foot-mouth disease in Hongkou district,2009-2018

目的分析2009-2018年虹口区手足口病疫情的流行病学及病原学特征,为防控措施提供依据。方法搜集2009-2018年虹口区手足口病疫情及病原学监测相关资料,并进行统计学分析。结果2009-2018年虹口区合计报告手足口病病例7697例,平均发病率84.46/10万,各年份发病率在40.37/10万~140.30/10万之间,年发病率差异有统计学意义(P<0.05)。全年呈现春夏季和秋季两个发病高峰,发病的主要人群为幼托和散居儿童(分别占47.95%和40.30%)。幼托机构是聚集性疫情的高发场所(65.96%)。2017和2018年柯萨奇病毒A16型(Cox A6)阳性比例分别达到73.33%和75.93%。结论加强对幼托机构学校等集体机构的手足口病预防控制措施可以有效控制疫情规模。Cox A6型已成为虹口区手足口病的优势型别,加强Cox A6型肠道病毒的监测非常必要。

Objective To analyze the epidemiological and etiological characteristics of hand-foot-mouth disease(HFMD)from 2009 to 2018 in Hongkou district,and provide evidence for the development of prevention and control strategies of HFMD.Methods HFMD cases and pathogens surveillance data from 2009 to 2018 were collected and statistical analysis was conducted.Results From 2009 to 2018,A total of 7697 HFMD cases of were reported in Hongkou district,with an average incidence of 84.46/10~5.The incidence of HFMD in each year ranged from 40.37/10~5 to 140.30/10~5,and the difference between annual incidence was statistically significant(P<0.05).There were the spring-summer and autumn two incidence peaks of HFMD through out the year in Hongkou district From 2009 to 2018.The main populations of HFMD cases were childcare and scattered children(47.95%and 40.30%).Childcares were the main sites of clustering cases(65.96%).The positive proportions of Coxsachie virus A 16(Cox A6)in 2017 and 2018 were 73.33%and 75.93%,respectively.Conclusions Strengthening the prevention and control measures of hand,foot and mouth disease in childcares,schools and other collective institutions could effectively control the scale of the epidemic.Cox A6 had become the dominant type of hand,foot and mouth disease in Hongkou district.It is necessary to strengthen the surveillance of Cox A6 enterovirus.
