摘要
目的研究分化型甲状腺癌(DTC)患者甲状腺球蛋白抗体(TgAb)对电化学发光法(ECLIA)和免疫放射法(IRMA)检测甲状腺球蛋白(Tg)的干扰程度,并观察不同实验对其干扰的检出能力。方法以2018年3月至2019年1月核工业四一六医院就诊的DTC患者和非DTC患者为研究对象,分别采用ECLIA和IRMA两种方法检测样本中的Tg,比较两种方法的差异。通过回收实验比较回收率差异以及TgAb对Tg检测的干扰。结果ECLIA法和IRMA法检测Tg比较:DTC组Z=-4.045,P<0.05;非DTC组Z=-1.748,P<0.05。回收率比较:DTC组Z=-2.309,P<0.05;非DTC组Z=-2.835,P<0.05。不同TgAb水平时Tg确证实验和回收实验方差分析结果分别为:DTC组:F=7.675,P<0.05;F=0.75,P=0.533。非DTC组:F=3.12,P<0.05;F=3.6761,P<0.05。结论确证实验较混合血清回收实验阳性率高;Tg被干扰的情况随TgAb水平增加而增大;TgAb在10~115 mIU/L时即可对Tg的测定造成干扰。ECLIA法较IRMA法更适合DTC患者手术治疗后的随访。
Objective To study the degree of interference of thyroglobulin antibody(TgAb)in patients with differentiated thyroid cancer(DTC)on the detection of thyroglobulin(Tg)by electrochemiluminescence(ECLIA)and immunoradiometry(IRMA),and to observe the different experimental approach for its ability to detect interference.Methods DTC patients and non-DTC cancer patients who were treated at the Nuclear Industry 416 Hospital from March,2018 to January,2019 were selected as the research subjects.Two methods,ECLIA and IRMA,were used to detect Tg in two groups of patients and then the results were compared.The recovery experiments was used to compare the recovery rate and the interference of TgAb on Tg detection.Results Tg comparison between ECLIA method and IRMA method was shown as following:Z=-4.045 in the DTC group,P<0.05;Z=-1.748 in the non-DTC cancer group,P<0.05;Z=-3.358 in all samples,P<0.05.The comparison between the confirmed experimental recovery rate and the recovery rate of the laboratory-made mixed serum was shown as following:DTC group Z=-2.309,P<0.05;non-DTC cancer group Z=-2.835,P<0.05.Analysis of variance of Tg recovery rate at different TgAb levels was shown as following:DTC group:confirmation experiment F=7.675,P<0.05,recovery experiment F=0.75,P=0.533;non-DTC cancer group:confirmation experiment F=3.12,P<0.05,recovery experiment F=3.6761,P<0.05.Conclusion The confirmation test has a higher positive rate than the mixed serum recovery test.The interference of Tg increases with the increase of TgAb concentration.TgAb can interfere with the determination of Tg at(10-115)mIU/L.The ECLIA method is more suitable than the IRMA method for the follow-up of DTC patients after surgical treatment.
