早期肠内营养支持对胃癌合并糖尿病患者术后血糖、营养状态及免疫状态的影响研究

Effects of Early Enteral Nutrition Support on Postoperative Blood Glucose, Nutritional Status and Immune Status of Patients with Gastric Cancer Complicated with Diabetes

目的:探究早期肠内营养支持对胃癌合并糖尿病患者术后血糖、营养状态及免疫状态的影响。方法:选取2017年1月-2019年3月于本院行胃癌根治术治疗的70例胃癌合并糖尿病患者为研究对象,依据术后营养支持方式的不同分为对照组与观察组,每组各35例。对照组行全肠外营养支持,观察组行早期肠内营养支持。比较两组术后不同时间的血糖水平、术前与术后3、7 d的营养状态及免疫状态指标。结果:观察组术后1-7 d的血糖水平均低于对照组(P<0.05)。术后3、7 d,观察组前清蛋白、转铁蛋白、总蛋白、IgA、IgM、IgG、CD3^+及CD4^+均高于对照组,而CD8^+均低于对照组(P<0.05)。结论:早期肠内营养支持可更好地稳定胃癌合并糖尿病患者术后血糖水平,改善营养状态及免疫状态,具有较高的应用价值。

Objective:To investigate the effects of early enteral nutrition support on postoperative blood glucose,nutritional status and immune status of patients with gastric cancer complicated with diabetes.Method:A total of 70 patients with gastric cancer complicated with diabetes underwent radical gastrectomy in our hospital from January 2017 to March 2019 were selected as the research objects.They were divided into control group and observation group according to different postoperative nutritional support methods,with 35 patients in each group.The control group was treated with total parenteral nutrition support,and the observation group was treated with early enteral nutrition support.Blood glucose levels at different times after surgery,nutritional status and immune status indexes before 3 and 7 d after surgery were compared between the two groups.Result:The blood glucose level at 1-7 d after surgery in the observation group were lower than those in the control group (P<0.05).3 and 7 d after surgery,the prealbumin,transferrin,total protein,IgA,IgM,IgG,CD3^+ and CD4^+ in the observation group were higher than those in the control group,while CD8^+ were lower than those in the control group (P<0.05).Conclusion:Early enteral nutrition support can better stabilize postoperative blood glucose level in patients with gastric cancer complicated with diabetes,improve nutritional status and immune status,and has higher application value.
