2型糖尿病患者幽门螺旋杆菌感染情况及胃肠病变特点

Helicobacter pylori infection and gastrointestinal characteristics in patients with type 2 diabetes mellitus

目的:探讨2型糖尿病感染幽门螺杆菌(Hp)的情况以及胃肠病变特征。方法:对行胃镜检查的140例患者的病历资料进行回顾性分析,根据患者是否合并2型糖尿病分成DM组与NDM组,每组各70例,对两组Hp感染情况及胃肠病变检出情况进行比较。结果:DM组Hp感染率为45.71%,显著高于NDM组的28.57%,差异有统计学意义(P<0.05);胃肠病变检出情况上,DM组在消化性溃疡、反流性食管炎与胃肠息肉的检出率上明显高于NDM组,差异有统计学意义(P<0.05);在检出的消化性溃疡上,DM组检出溃疡直径≥1.0cm的比例为71.43%,显著高于NDM组的30.00%,差异有统计学意义(P<0.05);两组在胃炎类型上差异无统计学意义(P>0.05),而检测慢性浅表性胃炎合并糜烂性胃炎上,DM组比例为59.70%,NDM组比例为36.92%,DM组显著高于NDM组,差异有统计学意义(P<0.05)。结论:2型糖尿病中感染Hp的几率高,且胃肠发病也显著。

Objective to investigate the status of helicobacter pylori(Hp)infection in type 2 diabetes mellitus and the characteristics of gastrointestinal diseases.Method retrospective analysis was performed on the medical records of 140 patients who underwent gastroscopy.Patients were divided into DM group and NDM group according to whether they had type 2 diabetes or not.70 patients in each group were divided into DM group and NDM group.Results The Hp infection rate in DM group was 45.71%,significantly higher than that in ndm28.57%(P<0.05).In terms of the detection of gastrointestinal diseases,the detection rate of peptic ulcer,reflux esophagitis and gastrointestinal polyps in the DM group was significantly higher than that in the NDM group(P<0.05).For the detected peptic ulcer,71.43%of the ulcer diameter≥1.0cm was detected in the DM group,significantly higher than that in the NDM group(P<0.05).There was no significant difference in the type of gastritis between the two groups(P>0.05),while the proportion of chronic superficial gastritis combined with erosive gastritis was 59.70%in the DM group and 36.92%in the NDM group,which was significantly higher than that in the NDM group(P<0.05).Conclusion The incidence of Hp infection in type 2 diabetes mellitus is high and gastrointestinal morbidity is significant.
