高频血管超声联合SMI技术对糖尿病足跖背动脉的应用研究 被引量:7

摘要 目的:探讨高频血管超声联合超微血管成像(superb microvascular imaging,SMI)技术对糖尿病足跖背动脉的应用价值。方法:选取2018年1月-2019年9月本院收治的糖尿病足患者80例(160侧)。所有患者均采用CDFI、SMI两种血流显像技术规范检查患者下肢动脉,采用高频超声及彩色多普勒血流显像(color doppler flow imaging,CDFI)方式得的数据归为A组,联合SMI血流显像方式的数据归为B组。比较两组第1、2跖背动脉血管显示率、管腔内径、阻力指数、血流速度;观察两组超声声像图特征。结果:B组第1、2跖背动脉的显示率均高于A组,差异均有统计学意义(P<0.05)。两组第1、2跖背动脉管腔内径、阻力指数、血流速度比较,差异均无统计学意义(P>0.05)。A组显示无血流,提示闭塞时,B组中部分病例能显示微弱血流信号或侧支循环。结论:高频CDFI联合SMI技术可清晰的显示糖尿病组患者跖背动脉管腔内径、血流速度及阻力指数,为临床诊断疾病提供重要价值,对指导临床早期干预、降低致残率具有重要意义。 Objective:To investigate the application value of high frequency vascular ultrasound combined with superb microvascular imaging(SMI)technology in dorsal metatarsal arteries of diabetic foot.Method:A total of 80 patients(160 sides)with diabetic foot admitted to our hospital from January 2018 to September 2019 were selected.All patients were examined by CDFI and SMI blood flow imaging techniques.The data of high frequency vascular ultrasound CDFI were included in A group,the data of CDFI combined with SMI were included in B group,and the ultrasonic features were observed.The vascular display rate,lumen diameter,resistance index and blood flow velocity of the first and second dorsal metatarsal arteries were compared between the two groups;observe the sonographic features of the two groups.Result:The display rate of the first and second metatarsal artery in group B was higher than that in group A,the difference was statistically significant(P<0.05).There was no significant difference in lumen diameter,resistance index and blood flow velocity between the two groups(P>0.05).No blood flow was shown in group A,indicating that some cases in group B could show weak blood flow signal or collateral circulation when occluding.Conclusion:High frequency vascular ultrasound combined with SMI technology can clearly display the lumen diameter,blood flow velocity and resistance index of dorsal metatarsal arteries in patients with diabetic foot,provide important value for clinical diagnosis of diseases,which is of great significance in guiding early clinical intervention and reducing disability rate.
