血液检验结果的影响因素及控制措施

Influencing Factors and Control Measures of Blood Test Results

目的调查血液检验结果的影响因素,总结相关的控制措施。方法回顾性分析2018年1月至2019年1月于凌海市人民医院接受血液检验的1000例患者的临床资料,通过应用Sysmex质控血浆和Sysmex CA7000全自动血浆分析仪进行相关的调查,配备的试剂盒为Thrombore凝血酶试剂盒。对于本研究患者的相关质控数据进行统计调查,分析凝血酶时间(TT)、活化部分凝血活酶时间(APPT)、凝血原时间(PT)、纤维蛋白原(FIB)等指标,并在24 h内对相关的检验标本进行持续性的10次检测,求平均值,分析偏倚情况。结果在1000例血浆标本的检验报告中,存在血浆检验结果不合格的标本66例(6.60%);不合格标本占比与合格标本占比相比,差异有统计学意义(P<0.05);因血液标本采集位置导致血液检验结果存在差异24例(36.36%),因样本检验不合格导致血液检验结果存在差异20例(30.30%),样本送检不合理占9例(13.64%),患者自身因素占7例(10.61%),其他6例(9.09%)。结论在进行血液检验的过程中,血液检验结果的影响因素主要表现在血液采集标本的位置不正确和样品检验不合格两个方面。临床需采取有效的质控措施进行干预,以提升标本检验的规范性和准确性。

Objective To investigate the factors influencing the accuracy of blood test results and summarize the relevant control measures.Methods In this paper,retrospective analysis of the clinical data of 1,000 patients who underwent blood tests at Linghai People’s Hospital from January 2018 to January 2019,through the application of Sysmex quality control plasma and Sysmex CA7000 automatic blood analyzer to relevant investigations,equipped with the kit for thrombore thrombin kit.For the purpose of this article related quality control data of patients treated by statistical investigation,analysis of part thrombin time(TT),activated clotting time(APPT)live enzymes,clotting time(PT)and fibrinogen(FIB),and other related indicators,to deduce accordingly according to the summary of imprecise density,and within 24 hours of the relevant test specimens for continuous ten times detection,averaging,analysis of bias.Results In the test report of 1000 plasma samples investigated in this paper,66 samples with unqualified plasma test results were found,accounting for 6.60%.Specifically,the location of blood sample collection led to differences in blood test results in 24 cases,accounting for 36.36%;unqualified sample inspection led to differences in blood test results in 20 cases,accounting for 30.30%;unreasonable sample submission accounted for 9 cases,accounting for 13.64%;patients'own factors accounted for 7 cases,accounting for 10.61%;other 6 cases,accounting for 9.09%.Conclusion In the process of blood test,factors that affect the accuracy of the blood test results of several aspects,mainly manifested in the location of the blood samples were taken two aspects of incorrect and sample inspection is unqualified,so clinical needs to take effective quality control measures to intervene,and promote the standardization of the test specimens.
