ICU专科护士小组在人工气道患者医疗器械相关性压力性损伤中的防控效果

The Prevention and Control Effect of ICU Specialist Nurse Team in Medical Device Related Pressure Injury of Artificial Airway Patients

目的:探讨ICU专科护士小组在人工气道患者医疗器械相关性压力性损伤(MDRPI)中的防控效果。方法:选取2019年1月-2020年1月在本院ICU住院治疗并留置人工气道的120例患者为研究对象。按照随机数字表法分为试验组和对照组,各60例。对照组由责任护士依据ICU-MDRPI的预防管理办法对留置人工气道患者进行人工气道相关压力性损伤的预防及护理,试验组由ICU专科护士小组成员对留置人工气道患者进行人工气道相关压力性损伤的防控管理。比较两组患者人工气道留置时间、意外拔管发生及再插管情况;比较两组患者人工气道MDRPI发生情况及护理满意度。结果:试验组人工气道MDRPI发生率为6.7%,低于对照组的20.0%,差异有统计学意义(P<0.05)。试验组患者人工气道平均留置时间短于对照组,意外拔管发生率、再插管率均低于对照组,差异均有统计学意义(P<0.05)。试验组患者的护理满意度为100%,高于对照组的88.3%,差异有统计学意义(P<0.05)。结论:ICU专科护士小组能提升人工气道患者MDRPI的防控管理效果,降低人工气道患者MDRPI发生率,应鼓励ICU专科护士积极参与临床实践。

Objective:To investigate the prevention and control effects of ICU specialist nurse team in medical device related pressure injury (MDRPI) of artificial airway patients.Method:From January 2019 to January 2020,120 patients who were hospitalized in ICU of our hospital and had artificial airway indwelling were selected as research objects.According to the random number table method,the patients were divided into the experimental group and the control group,60 cases in each group.The control group was given the prevention and nursing of artificial airway related pressure injury by the responsible nurse according to the prevention and management method of ICU-MDRPI.In the experimental group,members of the ICU specialist team of nurses carried out the prevention and control management of artificial airway related pressure injury in patients with indwared artificial airway.The indwelling time of artificial airway,the occurrence of accidental extubation and re-intubation,and the occurrence of MDRPI and nursing satisfaction were compared between the two groups.Result:The incidence of MDRPI in the experimental group was 6.7%,lower than 20.0% in the control group,the difference was statistically significant (P<0.05).The average indwelling time of artificial airway in the experimental group was shorter than that in the control group,the incidence of accidental extubation and the rate of re-intubation were lower than those in the control group,the differences were statistically significant (P<0.05).The nursing satisfaction of patients in the experimental group was 100%,higher than 88.3% of the control group,with statistically significant difference (P<0.05).Conclusion:ICU specialist nurses teams can improve the prevention,control and management effect of MDRPI in patients with artificial airway,reduce the incidence of MDRPI in patients with artificial airway,ICU specialist nurses should be encouraged to actively participate in clinical practice.
