母牛分枝杆菌对初治涂阴肺结核患者机体免疫水平的影响

Effect of Mycobacterium Vaccae on the Immune Level of Newly Treated Patients with Smear-negative Pulmonary Tuberculosis

摘要 目的:探讨母牛分枝杆菌对初治涂阴肺结核患者机体免疫水平及治疗效果的影响。方法:选取2014年12月-2016年4月于广东省江门市结核病防治机构就诊的100例涂阴肺结核患者为研究对象,将其随机分为对照组和试验组,每组50例。对照组按照2HRZE/4HR方案进行常规治疗,试验组采用母牛分枝杆菌+2HRZE/2HR方案进行治疗。比较两组治疗前后不同时期患者CD4+细胞和IgG免疫球蛋白水平,并观察两组的治疗效果。结果:有81例涂阴肺结核患者完成治疗,其中对照组40例,试验组41例。治疗6个月,试验组IgG水平高于对照组,差异有统计学意义(P<0.05);治疗2、6个月,试验组CD4+均高于对照组,差异均有统计学意义(P<0.05);经治疗后两组病灶均有很大程度的吸收,试验组疗效优于对照组,差异有统计学意义(P<0.05)。结论:母牛分枝杆菌可提高抗结核药物治疗初治涂阴肺结核的效果,能明显提高涂阴肺结核患者的机体免疫力,具有一定的临床意义。 Objective:To investigate the efficacy of Mycobacterium Vaccae in the treatment of smear-negative pulmonary tuberculosis and its effect on the immune level of patients.Method:From December 2014 to April 2016,100 patients with smear negative pulmonary tuberculosis who visited the tuberculosis prevention and control institution of Jiangmen City,Guangdong Province were selected as the research objects.They were randomly divided into the control group and the experimental group,with 50 patients in each group.The control group received routine treatment according to the 2HRZE/4HR protocol,the experimental group was treated with Mycobacterium Vaccae +2HRZE/2HR regimen.The levels of CD4^+ cells and IgG immunoglobulin in patients before and after treatment in the two groups were compared,and the therapeutic effects of the two groups were observed.Result:There were 81 patients with smear-negative pulmonary tuberculosis,including 40 in the control group and 41 in the experimental group.After 6 months of treatment,IgG level in the experimental group was higher than that in the control group,the difference was statistically significant (P<0.05).Treatment for 2,6 months,CD4^+ in the experimental group were higher than those in the control group,with statistically significant differences (P<0.05).After treatment,lesions in both groups were absorbed to a large extent,and the therapeutic effect of the experimental group was better than that of the control group,with statistically significant difference (P<0.05).Conclusion:Mycobacterium Vaccae can improve the curative effect of anti-tuberculosis drugs in the initial treatment of smear-negative pulmonary tuberculosis,and can significantly improve the body immunity of smear negative pulmonary tuberculosis patients,which has certain clinical significance.
