甲状腺疾病患者血清Tg-CIC中Tg-Ab水平检测及相关临床研究 被引量:1

Detection and Related Clinical Research of Tg-Ab in Serum Thyroglobulin Immune Complex(Tg-CIC)in Patients with Thyroid Diseases

摘要 目的对血清甲状腺球蛋白免疫复合物(thyroglobulin circulating immune complex,Tg-CIC)中抗甲状腺球蛋白抗体(anti-thyroglobulin antibody,Tg-Ab)含量检测,探讨Tg-CIC在甲状腺疾病中的临床以及流行病学意义。方法收集甲状腺疾病患者血清标本187例,每例血清标本分为两组(实验组和空白对照组),采用免疫复合物解离专利技术对甲状腺疾病患者血清标本Tg-CIC进行分离、解离,通过检测Tg-CIC解离后的Tg-Ab含量来间接反映甲状腺疾病患者血清中Tg-CIC水平,按照不同的甲状腺疾病进行分组并对实验结果分析讨论。结果不同甲状腺患者血清中Tg-CIC总阳性率达92.51%,不同甲状腺疾病血清中Tg-CIC的阳性率差异无统计学意义(χ²=2.917,P>0.05)。不同甲状腺疾病患者间血清游离Tg-Ab含量差异无统计学意义(H=3.882,P>0.05),不同甲状腺疾病患者间血清CIC中Tg-Ab含量差异无统计学意义(H=5.5842,P>0.05)。不同甲状腺疾病患者中,甲状腺功能亢进、甲状腺炎以及甲状腺结节患者血清游离Tg-Ab的含量与血清CIC中的Tg-Ab含量呈正相关(P<0.05),甲状腺功能减退、甲状腺肿瘤患者血清游离Tg-Ab的含量与血清CIC中的Tg-Ab含量不相关(P>0.05)。结论在甲状腺疾病患者血清中,大部分可检出Tg-CIC,与疾病类型无关。而甲状腺疾病患者中,甲状腺功能亢进、甲状腺炎以及甲状腺结节患者血清游离Tg-Ab含量与血清CIC中Tg-Ab含量存在相关性,为我们进一步了解Tg-CIC与甲状腺疾病的发生、发展的相关性研究奠定了基础,提供了新的研究视角。 Objective To detect the content of anti-thyroglobulin antibody(Tg-Ab)in serum thyroglobulin immune complex(Tg-CIC)and study the clinical and epidemiological significance of Tg-CIC in thyroid diseases.Methods 187 serum samples of patients with thyroid diseases were collected,and each serum sample was divided into two groups(experimental group and blank control group)thyroid diseases were divided into groups and the experimental results were analyzed and discussed.Results The total positive rate of Tg-CIC in patients with different thyroid diseases was 92.51%.There was no significant difference in the positive rate of Tg-CIC in different thyroid diseases(χ²=2.917,P>0.05).There was no significant difference in serum free Tg-Ab content between patients with different thyroid diseases(H=3.882,P>0.05),and there was no significant difference in serum Tg-Ab content in CIC among patients with different thyroid diseases(H=5.5842,P>0.05).In patients with different thyroid diseases,the content of serum free Tg-Ab in patients with hyperthyroidism,thyroiditis and thyroid nodules was positively correlated with the content of Tg-Ab in serum CIC(P<0.05).The content of serum free Tg-Ab in patients with hypothyroidism and thyroid tumor was not correlated with that in serum CIC(P>0.05).Conclusion Tg-CIC could be detected in the serum of patients with thyroid disease,which had nothing to do with the type of disease.In patients with thyroid diseases,the content of serum free Tg-Ab in patients with hyperthyroidism,thyroiditis and thyroid nodules was correlated with the content of Tg-Ab in serum CIC,which lays a foundation and provides a new research perspective for us to further understand the correlation between Tg-CIC and the occurrence and development of thyroid diseases.
