温阳化气法治疗糖尿病合并下肢动脉硬化闭塞症效果观察

Observation on the effect of therapeutic method of warming Yang and dissipating Qi on diabetes with lower limb arteriosclerosis obliterans

目的观察温阳化气法治疗糖尿病合并下肢动脉硬化闭塞症的临床疗效。方法选择2018年1月—2019年1月沧州中西医结合医院收治的糖尿病合并下肢动脉硬化闭塞症患者100例,使用随机数字法将患者分为观察组和对照组各50例。对照组患者给予常规对症治疗及前列地尔静滴,观察组在对照组治疗方案基础上以温阳化气法为治则,给予复荣通脉组方治疗,2组均以14 d为1个疗程,疗程间间隔3 d,治疗2个疗程。比较2组患者治疗前后中医症状评分、血清炎性指标和患肢血流情况,评估2组临床疗效。结果2组治疗后中医症状评分均低于治疗前(P均<0.05),且观察组中医症状评分低于对照组(P<0.05)。2组治疗后C反应蛋白(CRP)、白细胞介素-6(IL-6)水平均低于治疗前(P均<0.05),且观察组CRP、IL-6水平低于对照组(P均<0.05);2组患者治疗后患肢股动脉、腘动脉、胫前动脉、足背动脉血流峰速均高于治疗前(P均<0.05),且观察组各指标均高于对照组(P均<0.05)。观察组治疗有效率为96%(48/50),明显高于对照组的84%(42/50)(P均<0.05)。结论温阳化气法治疗糖尿病合并下肢动脉硬化症疗效确切,能够显著改善患者临床症状,加快下肢动脉血液流速,降低血清炎症指标。

Objective It is to observe the clinical curative effect of therapeutic method of Wenyang Huaqi on diabetes mellitus complicated with lower extremity arteriosclerosis obliterans.Methods 100 patients with diabetes and lower extremity arteriosclerosis obliterans treated in Cangzhou Hospital of Integrated Traditional Chinese and Western Medicine from January 2018 to January 2019 were randomly divided into observation group and control group by random number method,50 cases in each group.The patients in the control group were treated with conventional therapy and Alprostadil injection by intravenous drip,and the patients in the observation group were treated with Furong Tongmai Decoction which according to the treatment method of warming Yang and transforming qi on the basis of the treatment plan of the control group.14 days as a course of treatment,after 3 days of drug withdrawal,another courses of treatment was continued.The scores of TCM symptoms,serum inflammatory indexes and blood flow of affected limbs before and after treatment were compared between the two groups,and the clinical curative effect were evaluated.Results After treatment,the TCM syndrome scores of the patients in the two groups were significantly lower than those before treatment(P<0.05),and the TCM syndrome scores of the observation group were lower than those of the control group(P<0.05).The levels of C-reactive protein(CRP)and,interleukin-6(IL-6)after treatment were significantly lower than those before treatment in the two groups(P>0.05),and the levels of CRP and IL-6 of the observation group were significantly lower than those of the control group(P<0.05).The blood flow peak velocity levels of femoral artery,popliteal artery,anterior tibial artery,dorsal foot artery of affected limbs after treatment were significantly higher than those before treatment in the two groups(P>0.05),and the levels of these indexes in the observation group were higher than those in the control group(P<0.05).The effective rate of treatment in the observation group was 96%(48/50),which was significantly higher than 84%(42/50)in the control group(P<0.05).Conclusion The treatment method of warming Yang and dissipating Qi has a definite curative effect in the treatment of diabetes complicated with arteriosclerosis of the lower extremities.It can significantly improve the clinical symptoms,increase the arterial blood flow rate of the lower extremities,and reduce serum inflammation indicators.
