摘要
目的探讨miRNA-21在多发性骨髓瘤患者骨髓中的表达及与预后的相关性。方法收治多发性骨髓瘤患者62例作为MM组,选择同期行骨髓捐献的健康人员62例作为对照组,取所有入选者的骨髓样本,采用qPCR方法检测miRNA-21表达水平,调查入选者的临床资料,随访患者的预后并进行相关性分析。结果MM组miRNA-21相对表达量显著高于对照组(P<0.05)。MM组血清免疫球蛋白(Ig)G、IgA、β2微球蛋白(β2-MG)值高于对照组,乳酸脱氢酶(LDH)、白蛋白(ALB)、血红蛋白(Hb)值低于对照组,对比差异都有统计学意义(P<0.05)。MM组患者随访至今(2020年3月1日),平均随访时间(32.72±2.28)个月,死亡18例,死亡率为29.0%。直线相关分析显示:多发性骨髓瘤患者的预后死亡与miRNA-21、β2-MG、IgA、临床分期呈正相关性(P<0.05),与ALB、LDH、Hb呈负相关性(P<0.05)。COX回归分析显示:miRNA-21、临床分期、β2-MG、Hb、ALB为影响多发性骨髓瘤患者预后的主要因素(P<0.05)。结论miRNA-21在多发性骨髓瘤患者骨髓中呈现高表达状况,与患者的血液学指标显著相关,可影响与预测患者的预后。
Objective To investigate the expression of miRNA-21 in bone marrow of patients with multiple myeloma and its correlation with prognosis.Methods Sixty-two patients with multiple myeloma were admitted as the MM group,and 62 healthy individuals who underwent bone marrow donation during the same period were selected as the control group.Bone marrow samples were detected for miRNA-21 expression level by qPCR method.The clinical data of the selected patients were investigated.The prognosis of the patients were followed up and the correlation analysis were performed.Results The relative expression of miRNA-21 in the MM group were significantly higher than that in the control group(P<0.05).The serum immunoglobulin(Ig)G,IgA,β2 microglobulin(β2-MG)values in the MM group were higher than those in the control group,and lactate dehydrogenase(LDH),albumin(ALB),and hemoglobin(Hb)values were lower The control group that compared were statistically significant differences(P<0.05).The patients in the MM group were followed up to date(March 1,2020),with the average follow-up time were 32.72±2.28 months,there were 18 patients were died,and the mortality rates were 29.0%.Linear correlation analysis showed that the prognosis of patients with multiple myeloma were positively correlated with miRNA-21,β2-MG,IgA,and clinical stage(P<0.05),and were negatively correlated with ALB,LDH,and Hb(P<0.05).COX regression analysis showed that miRNA-21,clinical stage,β2-MG,Hb,ALB were the major factors affected the prognosis of patients with multiple myeloma(P<0.05).Conclusion MiRNA-21 are highly expressed in the bone marrow of patients with multiple myeloma,which are significantly related to the hematological parameters of patients,and can affect and predict the prognosis of patients.
