摘要
目的探究NACT联合手术治疗早中期宫颈癌的效果及与病理组织中COX-2表达的关系。方法选取早中期宫颈癌患者128例,按随机数字表法将其分为对照组(n=64)和观察组(n=64)。对照组给予单纯手术治疗,观察组在此基础上联合NACT治疗。统计2组术后临床疗效;比较2组患者术后转移率;比较2组患者术后肿瘤标志物情况;比较2组患者术后不良反应发生情况;比较化疗后观察组与对照组宫颈组织COX-2蛋白表达情况。结果术后观察组临床总有效率显著高于对照组(P<0.05)。观察组术后切缘阳性率、脉管浸润率、盆腔淋巴结转移率、宫旁累及率显著低于对照组(P<0.05)。术后观察组SCC-Ag、CYFRA21-1水平显著低于对照组(P<0.05)。观察组化疗后宫颈组织COX-2蛋白的表达阳性率明显低于对照组(P<0.05)。结论NACT联合手术可有效提高治疗有效率,降低早中期宫颈癌患者术后转移率及肿瘤标志物表达水平,改善患者预后,作用机制可能与其能有效降低宫颈癌组织COX-2表达相关。
Objective To explore the effect of NACT combined with surgery on the prognosis of early and middle stage cervical cancer patients and the relationship with COX-2 expression in pathological tissues.Methods 128 patients with early and middle stage cervical cancer were randomly divided into the control group(n=64)and the observation group(n=64).The control group was treated with surgery alone,and the observation group was treated with NACT on this basis..Statistical analysis of the clinical efficacy of the 2 groups;postoperative metastasis rates in the 2 groups were compared;comparison of the 2 groups of tumor markers after surgery;comparison of adverse reactions after surgery in the 2 groups;comparison of COX-2 protein expression in cervical tissues between the 2 groups after operation.Results After operation,the total clinical effective rate of the observation group was memorably higher than that of the control group(P<0.05);after operation,positive rate of incision margin,vascular infiltration rate,pelvic lymph node metastasis rate and parauterine involvement rate in the observation group were substantially lower than those in the control group.(P<0.05);after operation,the level of SCC-Ag and CYFRA21-1 in the observation group were dramatically lower than those in the control group(P<0.05);after operation,the positive rate of expression in cervical tissues of COX-2 protein in the observation group was markedly lower than that in(P<0.05).Conclusion NACT combined with surgery can effectively reduce the metastasis rate and the expression of tumor markers in early and middle stage cervical cancer patients,and improve the prognosis of patients.Its mechanism may be related to the effective reduction of COX-2 expression in cervical cancer tissues.
