经尿道钬激光切除术对非浸润性膀胱癌患者术后OPNI、NLR及血清CXCL5、尿NMP22水平的影响

The Effect of Transurethral Holmium Laser Resection on Postoperative OPNI、NLR and Serum CXCL5、Urine NMP22 in Non-invasive Bladder Cancer Patients

目的探讨经尿道钬激光切除术(HOLRBT)对非浸润性膀胱癌(NMIBC)患者术后营养指数(OPNI)、中性粒细胞与淋巴细胞比值(NLR)及血清趋化因子配体5(CXCL5)、尿细胞核基质蛋白22(NMP22)水平的影响。方法选择NMIBC患者88例,按随机数字表法分为两组各44例。对照组行经尿道膀胱肿瘤电切术(TURBT),研究组行HOLRBT。对比两组围术期指标、手术前后OPNI、NLR、CXCL5、尿NMP22水平,并发症及复发率。结果研究组手术时间、术毕膀胱冲洗时间、尿管留置时间、住院时间均短于对照组,术中出血量少于对照组,差异有统计学意义(P<0.05);研究组术后OPNI水平高于对照组,NLR、血清CXCL5、尿NMP22水平低于对照组,差异有统计学意义(P<0.05);研究组并发症发生率低于对照组,差异有统计学意义(P<0.05);两组术后1年复发率比较,差异无统计学意义(P>0.05)。结论HOLRBT可有效改善NMIBC病情,创伤小、出血量少、住院时间短,利于术后恢复,同时降低血清肿瘤标志物水平,利于促进病情转归。

Objective To investigate the effects of transurethral holmium laser resection(HOLRBT)on nutritional index(OPNI),neutrophil-to-lymphocyte ratio(NLR),serum chemokine ligand 5(CXCL5)and urinary nuclear matrix protein 22(NMP22)levels in patients with non-invasive bladder cancer.Methods 88 NMIBC patients according to the method of random number table,were divided into 2 groups,each with 44 cases.The control group underwent transurethral resection of bladder tumor(TURBT),of the study group received HOLRBT.To compare the perioperative indexes,OPNI、NLR、CXCL5、urine NMP22 before and after operation,Complications and recurrence rates.Results The operation time,bladder irrigation time,catheter retention time and hospitalization time were shorter than those of the control group,Less intraoperative bleeding than the control group,difference was statistically significant(P<0.05);OPNI levels in the study group were higher than those in the control group,NLR、serum CXCL5、urine NMP22 was lower than the control group,difference was statistically significant(P<0.05);The incidence of complications in the study group was lower than that in the control group,The difference was statistically significant(P<0.05);there was no significant difference in recurrence rate between the two groups(P<0.05).Conclusion HOLRBT can effectively improve the condition of NMIBC,small trauma,less bleeding,short hospitalization time,which is conducive to postoperative recovery,and reduce the level of serum tumor markers,and promote the outcome of the disease.
