摘要
[目的]探讨心肺综合护理干预对老年慢性心力衰竭患者心肺功能的影响。[方法]将124例老年慢性心力衰竭患者分为两组,每组62例。对照组给予常规慢性心力衰竭治疗和护理,观察组予以强化心肺康复综合护理干预,观察6 mon。干预前后检测两组患者的左室射血分数、6 min步行距离、1 s内最大呼气量、用力肺活量、脑钠肽水平评估心肺功能变化;采用KCCQ评定生存质量。[结果]入院时两组的左室射血分数、6 min步行距离、1 s内最大呼气量、用力肺活量、脑钠肽水平、KCCQ评分比较差异无统计学意义(P>0.05)。经干预6 mon后,观察组的左室射血分数、6 min步行距离、1 s内最大呼气量、KCCQ评分显著高于对照组,脑钠肽水平显著低于对照组,差异均有统计学意义(P<0.05)。用力肺活量无明显差异(P>0.05)。[结论]对老年慢性心力衰竭患者在常规治疗和护理的基础上予以强化心肺康复综合护理干预,能有效改善患者的心肺功能,显著提高生存质量。
[Objective]To explore the influence of comprehensive nursing intervention on cardio-pulmonary functions of senile patients with chronic heart failure(CHF).[Methods]A total of 124 senile CHF patients were assigned to two groups(both n=62).Control group received routine CHF treatment and nursing,observation group did intensified cardio-pulmonary rehabilitation comprehensive nursing intervention,and they were observed for 6 months.Such indexes were detected before and after intervention as left ventricular ejection fraction(LVEF),6 minute walking distance(6 MWD),one-second forced expiratory volume(FEV1),forced vital capacity(FVC)and brain natriuretic peptide(BNP)to evaluate the changes of cardio-pulmonary functions;Qualities of life were assessed with Kansas City Cardiomyopathy Questionnaire(KCCQ).[R esults]There were no group differences in scores on the LVEF,6MWD,FEV1,FVC,BNP and KCCG on admission(P>0.05).After 6 month interventions the LVEF,6MWD,FEV1 and.KCCG score were significantly higher and BNP was lower in observation than in control group,differences had statistical significance(P<0.05);the FVC had no notable group difference(P>0.05).[Conclusion]Intensified cardio-pulmonary reha bilitation comprehensive nursing intervention on the basis of routine treatment and nursing could effectively improve patients’cardio pulmonary functions and life quality of senile CHF patients.
