2型糖尿病合并结直肠癌患者血清Chemerin的表达及意义

Expression and clinical significance of serum Chemerin in patients with type 2 diabetes complicated with colorectal cancer

目的探讨血清中脂肪因子Chemerin水平与2型糖尿病(type 2 diabetes mellitus,T2DM)合并结直肠癌(colorectal cancer,CRC)的相关性,进一步探索2型糖尿病相关的炎症相关因子与结直肠癌的关系,明确结直肠癌发病的重要炎症基础,这对于肿瘤的预防有重要的意义,也为降低2型糖尿病人群患结直肠癌的风险提供的新的预防方法。方法选取2019年5月—2020年5月河南省人民医院收治的126例2型糖尿病患者根据是否合并结直肠癌分成两组:未合并结直肠癌组61例,合并结直肠癌组65例,采用酶联免疫吸附法检测两组的血清Chemerin及相关因子水平。结果 (1)合并结直肠癌的2型糖尿病患者的血清Chemerin水平明显高于未合并结直肠癌的2型糖尿病患者(P<0.05)。(2)2型糖尿病合并结直肠癌患者血清中CRP及Chemerin表达量高于单纯2型糖尿病患者(P<0.05)。(3)血清Chemerin水平可能是影响2型糖尿病患者发生结直肠癌的一个危险因素。结论血清CRP等炎症因子参与胰岛素抵抗过程,2型糖尿病合并结直肠癌患者致癌因素与脂肪因子促进炎性反应的作用有关,Chemerin的异常表达与2型糖尿病合并结直肠癌患者疾病的发生发展以及预后有密切关系。血清Chemerin水平或许可以作为预测2型糖尿病合并结直肠癌发生发展的一个重要血清学指标。

Objective To investigate the correlation between serum adipocytokine Chemerin level and type 2 diabetes(type 2 diabetes mellitus,T2 DM)complicated with colorectal cancer(colorectal cancer,CRC).Methods A total of 126 patients with type 2 diabetes were randomly divided into two groups according to whether they had colorectal cancer or not:61 patients in the group without colorectal cancer and 65 patients in the group with colorectal cancer, serum Chemerin and related factors were detected by ELISA levels in the two groups.Results(1)The serum Chemerin level of type 2 diabetic patients with colorectal cancer was significantly higher than that of type 2 diabetic patients without colorectal cancer(P<0.05).(2)The serum expressions of CRP and Chemerin in patients with type 2 diabetes combined with colorectal with colorectal cancer were higher than those in patients with type 2 diabetes alone(P<0.05).(3)Serum Chemerin level may be a risk factor for colorectal cancer in patients with 2 diabetes.Conclusion Serum CRP and other inflammatory factors are involved in insulin resistance, the carcinogenic factors in the patients with type 2 diabetes complicated with colorectal cancer are related to the role of fatty factors in promoting inflammatory response,abnormal expression of Chemerin is closely related to the occurrence,development and prognosis of type 2 diabetes patients with colorectal cancer.Serum Chemerin level may be an important serological indicator to predict the occurrence and development of type 2 diabetes complicated with colorectal cancer.
