摘要 目的探讨妊娠期糖尿病对新生儿心脏发育的影响。方法选取2017年11月到2019年11月平顶山市第一人民医院产妇113例,符合国际糖尿病与妊娠研究组GDM诊断标准的足月新生儿为GDM组(53例);同期收治的非GDM母亲分娩的足月新生儿为对照组(60例)。比较两组一般指标:体重、身长、胎盘重量、性别、胎龄;心脏测量数据:主动脉宽度、左心房、右心室、主肺动脉宽度、左心室和室间隔厚度、射血分数和左室短轴短缩率;生化指标:糖化血红蛋白、糖化白蛋白、胰岛素。结果两组患者在胎龄、射血分数和左室短轴短缩率方面比较,差异无统计学意义(P>0.05);GDM组出生体重、身长、胎盘重量均大于对照组,差异具有统计学意义(P<0.05);GDM组主动脉宽度、左心房、右心室、主肺动脉宽度、左心室和室间隔厚度均大于对照组,差异具有统计学意义(P<0.05);GDM组糖化血红蛋白、糖化白蛋白、胰岛素水平均高于对照组,差异有统计学意义(P<0.05)。结论 GDM对新生儿的心脏发育有明显的不良影响。 Objective To explore the effect of gestational diabetes mellitus(GDM)on neonatal heart development.Methods Methods Between November 2017 and November 2019,113 pregnant women from the First People′s Hospital of Pingdingshan City were enrolled,of which full-term neonates met the GDM diagnostic criteria of the International Association of Diabetes and Pregnancy Study Groups(IADPSG)were as the GDM group(53 cases),and the term neonates delivered by non-GDM mothers within the same period were as the control group(60 cases).The general indicators:weight,length,placental weight,gender,gestational age;cardiac measurement data:aortic width,left atrium,right ventricle,main pulmonary artery width,left ventricular and ventricular septum thickness,ejection fraction,and left ventricular short axis short contraction;biochemical indicators:glycated hemoglobin,glycated albumin,insulin were compared in both groups.Results There was no significant difference in gestational age,ejection fraction and left ventricular short axis short-shrinkage rate between the two groups(P>0.05).The birth weight,length,and placental weight of the GDM group were greater than those of the control group(P<0.05).The aortic width,left atrium,right ventricle,width of aortic artery,thickness of left ventricle and interventricular septum in the GDM group were larger than those in the control group(P<0.05).The levels of glycated hemoglobin,glycated albumin,and insulin in the GDM group were higher than those in the control group(P<0.05).Conclusion GDM has a significant adverse effect on neonatal cardiac development.
